- NA This is a combined synopsis/solicitation for commercial items prepared IAW the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation Number DADA13-02-Q-0050 applies and is issued as a Request for Quote (RFQ). Solicitation Line Item is as follows: Line Item 0001, Ortho Tri-Cyclen, Mfr: Ortho-McNeil Pharmaceuticals Inc., NDC: 00062-1903-20, (144 Cycles/Case), 36 Cases; Required Delivery Date 17 January 2002, or please state earliest delivery schedule. Request FOB destination pricing. Delivery address will be Madigan Army Medical Center, Bldg. 9040A (Receiving Sect) Fitzsimmon s Drive, Tacoma WA 98431. This RFQ is issued for full and open competition with an NAICS code of 325412 and a size standard of 500 employees. The following clauses apply to this solicitation: FAR 52.212-1, Instructions to Offerors-Commercial Items; howev er sub-paragraphs (g) and (h) are deleted as this is an RFQ. FAR 52.212-2, Evaluations, Commercial Items with the following factors included in FAR 52.212-2(a) used to evaluate offers: Technical, Past Performance, Required Delivery Date and Price. Techni cal, Past Performance and Required Delivery Date, when combined, are significantly more important than price. Technical shall be evaluated based on the ability to obtain the required product. If quoter is not an authorized distributor of the product descr ibed, explain where and how the company intends to obtain them to meet the delivery schedule. The Government reserves the right to award to other than the low bidder. Quoters shall submit a minimum of two references with whom past performance can be verif ied to include points of contact, telephone numbers and total award amount. Quoters shall submit as a part of their quote, completed copies of the provisions at FAR 52.212-3 with Alt I, Offeror Representations and Certifications-Commercial Items and DFARS 252.212-7000, Offeror Representations and Certifications-Commercial Items. FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition; addenda are stated herein. FAR 52.212-5, Contract Terms and Conditions Required to Imple ment Statutes or Executive Orders-Commercial Items applies to this acquisition. The following clauses within paragraph 52.212-5(b) apply to this solicitation: 52.222-21, Prohibition of Segregated Facilities; 52.222-26, Equal Opportunity; 52.222-35, Affir mative Action for Disable Veterans and Veterans of the Vietnam Era; 52.222-36, Affirmative Action for Workers with Disabilities; 52.222-37, Employment Reports on Disable Veterans and Veterans of the Vietnam Era; 52.222-19, Child Labor-Cooperation with Auth orities and Remedies; 52.225-13, Restriction of Certain Foreign Purchases; and 52.232-33, Payment by Electronic Funds Transfer-Central Contractor Registration. DFARS 252.204-7004, Required Central Contractor Registration and DFARS 252-212-7001, Contract T erms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items apply to this acquisition. The following clauses within DFARS paragraph 252.212-7001(a) apply to this solicitation: 252-225-7001 , Buy American Act and Balance of Payments Program; 252.225-7012, Preference for Certain Domestic Commodities; and 252.247-7023, Transportation of Supplies by Sea. Quotes in response to this combined synopsis/solicitation are due not later than 26 December 2001, 2:00 PM Pacific Time. Once the de termination has been made by the Contracting Officer as to with whom a contract will be entered, a bilateral contract requiring the quoter?s signature will be i ssued. A contract will be in existence once both the quoter and the Contracting Officer have signed the document. Wherever the words ?offer?, ?proposal?, ?offerors?, or similar terms are used in this solicitation, they shall be read to mean ?quote?, ?qu otation?, or ?quoter? or similar corresponding terms to reflect that this solicitation is a Request for Quote. Firms offering in response to this notice should ensure registration in the Central Contractor Registration Database, prior to submission for co nsideration.
