- NA This is a combined synopsis/solicitation for commercial items prepared IAW the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation Number DADA13-02-T-0015 applies and is issued as a Request for Quote (RFQ). Solicitation Line Item is as follows: Line Item 0001, Imaging Table System, P/N ITS (includes retractable ?I? base f or modular table system, P/N 5890 and Radiolucent Imaging Top w/Gimbals, 500 lbs pt weight capacity, P/N 5927), 1 ea; Line Item 0002, Pivoting Arm Board w/3? ShearGuard Pad, P/N 5357, 2 ea; Line Item 0003, Side Rail Adapter for Orthopedic and Imaging Top, P/N 5855-830, 2 ea. Items must be compatible with the Jackson Spine Top, Radiolucent Imaging Top and the Orthopedic Fracture Table Top from Orthopedic Systems Inc. Required Delivery Date 15 January 2002, or please state earliest delivery schedule. Reque st FOB destination pricing. Delivery address will be Madigan Army Medical Center, Bldg. 9040A (Receiving Sect) Fitzsimmons Drive, Tacoma WA 98431. This RFQ is issued for full and open competition with an NAICS code of 425150 and a size standard of 500 emp loyees. The following clauses apply to this solicitation: FAR 52.212-1, Instructions to Offerors-Commercial Items; however sub-paragraphs (g) and (h) are deleted as this is an RFQ. FAR 52.212-2, Evaluations, Commercial Items with the following factors in cluded in FAR 52.212-2(a) used to evaluate offers: Technical, Past Performance, Required Delivery Date and Price. Technical, Past Performance and Required Delivery Date, when combined, are significantly more important than price. Technical shall be evalu ated based on the ability to function as required and compatibility as mentioned. The Government reserves the right to award to other than the low bidder. Quoters shall submit a minimum of two references with whom past performance can be verified to inclu de points of contact, telephone numbers and total award amount. Quoters shall submit as a part of their quote, completed copies of the provisions at FAR 52.212-3 with Alt I, Offeror Representations and Certifications-Commercial Items and DFARS 252.212-700 0, Offeror Representations and Certifications-Commercial Items. FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition; addenda are stated herein. FAR 52.212-5, Contract Terms and Conditions Required to Implement Statute s or Executive Orders-Commercial Items applies to this acquisition. DFARS 252.204-7004, Required Central Contractor Registration and DFARS 252-212-7001, Contract Terms and Con ditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items apply to this acquisition. The following clauses within DFARS paragraph 252.212-7001(a) apply to this solicitation: 252-225-7001, Buy Americ an Act and Balance of Payments Program; 252.225-7012, Preference for Certain Domestic Commodities; and 252.247-7023, Transportation of Supplies by Sea. Quotes in response to this combined synopsis/soli citation are due not later than 26 December 2001, 2:00 PM Pacific Time. Once the determination has been made by the Contracting Officer as to with whom a contract will be entered, a bilateral contract requiring the quoter?s signature will be issued. A co ntract will be in existence once both the quoter and the Contracting Officer have signed the document. Wherever the words ?offer?, ?proposal?, ?offerors?, or similar terms are used in this solicitation, they shall be read to mean ?quote?, ?quotation?, or ?quoter? or similar corresponding terms to reflect that this solicitation is a Request for Quote. Firms offering in response to this notice should ensure registration in the Central Contractor Registration Database, prior to submission for consideration.
