Description

This is a solicitation notice for commercial items prepared in accordance with the subparts in Federal Acquisition Regulation 12.6. Solicitation number Request for Quotation (RFQ) 673-41-02 is assigned and shall be referenced on any request for solicitation. Solicitation shall be issued on or about Debember 21, 2001. The James A. Haley Veterans Hospital, 13000 Bruce B Downs Blvd, Tampa, FL is seeking Pharmacist Services at the Brevard VA Outpatient Clinic, 2900 Veterans Way, Viera, FL for the period of January 14, 2002, or date of award, whichever is later, through June 30, 2002. The contractor-supplied Pharmacist shall be registered and licensed in a State, Territory or Commonwealth of the United States or District of Columbia. The work hours are Tuesday, Thursday and Friday, 10:00am through 4:30pm, wxcluding 1/2 hour for lunch and excluding national holidays. The NAICS Code is 621999 and the Size Standard is $5.0 million. This is a 100% Total Small Business Set Aside. See numbered note #1. Requests for solicitation package must be in writing or via fax at 813-903-4838. Pending contract is subject to the availability of funds.
