- NA Provide Counseling Service for approximately 1,134 federal employees for one base year plus three option years for the Louisville District Corps of Engineers. Solicitation will be issued o/a 23 January 2002 with a due date of 15 February 2002. CHARACTER AND EXTENT OF SERVICES: The contractor shall furnish all services, materials, equipment, supplies, plant, labor, transportation (including fuel), superintendence and professional supervision, and perform all work as necessary to provide an Employee Assist ance Program to the Louisville District Corps of Engineers as described in the Scope of Service, the performance of which shall be in strict accordance with same. METHOD OF PAYMENT: Payment will be made at quarterly intervals in arrears upon receipt of an invoice. Payment shall be made to the contractor based on the number of employees covered by this contract, regardless of the number of employees counseled. REQUIRED INSURANCE: The contractor shall procure and maintain during the entire period of perfo rmance under this contract the following minimum insurance. Type: General Liability - $1,000,000.00. In addition, the contractor shall carry Indemnification and Medical Liability Insurance. SCOPE OF SERVICES: The contractor will provide a complete di agnostic and referral service to maintain the Louisville area Federal Employees Assistance Program, including field employees. These services are to be provided to the immediate family members of employees covered by the program, whenever such services ar e offered, at no additional cost to the Government. The contractor shall employ appropriate staff necessary to provide services for approximately 1,134 employees of the Louisville District Corps of Engineers. This program is an integral part of overall oc cupational health; however, it will have no direct connection with existing Health Unit maintained by the Louisville District Corps of Engineers. CHANGES IN THE SCOPE OF SERVICES: The right is reserved, as the interest of the Government may require, to rev ise or amend the Scope of Services prior to the date set for receipt of proposals. Such revisions and amendments, if any, will be announced by amendment or revision to this solicitation. Copies of such amendments as may be issued will be furnished to all prospective offerors. If the revision and/or amendment is of a nature which require material changes in quantities or prices proposed, or both, the date set for receipt of proposals may be postponed in order to provide offerors with sufficient time to re vise their proposals. In such cases, the amendment will include an announcement of the new date for receipt of proposals. EVALUATION FACTORS FOR AWARD: Proposals will be evaluated in accordance with the following criteria, listed in descending order of importance: (a) Methodology;(b)Quality and Size of Staff; (c) Past Performance and (d) Total Price.
