- MARKET SURVEY AIRMAN CERTIFICATION CERTIFICATES The Federal Aviation Administration (FAA) is conducting a market survey for the purpose of determining the extent of small and/or small, disadvantaged vendors, so certified by the Small Business Administration under Section 8(a) of the Small Business Act (15 U.S.C. 637(a)), that exist in the State of Oklahoma, possessing the ability to provide the Civil Aviation Registry, Airmen Certification Branch (AFS-760)Flight Standards with airman certificates to be produced on plastic card stock media. Certificates must be approximately 3.5" by 2", containing information inclusive of airman name and address, personal information, certificate number, ratings held, the Department Of Transportation logo, the FAA Administrator's signature, and space for the airman's signature, at a minimum. Security features must be included in the design. Contractor must possess the ability to receive and process electronic input files, provided in an ASCII text format, comprised of approximately 600 - 3500 certificate records daily, containing all information to be included for each certificate. After each daily printing process, contractor will be required to mail certificates and provide to the FAA daily electronic reports as to certificates mailed. Complete flexibility would be required for edit and/or return to the FAA of any certificate, at the FAA's request prior, to daily approval and mailing. After daily mailings are accomplished, all electronic records must be destroyed. Quality control and prompt product delivery are paramount. Daily interaction with federal personnel will be required, therefore the contractor will be required to have an office located in the local Oklahoma City commuting area at the time of contract commencement. Standard Industrial Classification applicable hereto are inclusive of 7374 (NASIC 514210) and 2759 (NASIC 323119). Prospective offerors must have a minimum of three years experience in similar type contracts, providing product and support services, within the past five years on a minimum of 2 prior relevant contracts of over $100,000 per year, employing a minimum of 10 employees with multiple tasks. INTERESTED CONTRACTORS SHOULD INCLUDE WITH THEIR RESPONSE THE FOLLOWING INFORMATION AT A MINIMUM: 1) Contract experience detailing the above as required. Supporting documentation must provide contract numbers and dates, company names, phone numbers, points of contact, dollar values, and task information in sufficient detail to determine compliance with above requirements. 2) Copy of SB/SEDB certifications, including dates, as certified by the SBA. Interested companies should submit all detailed information set forth above no later than February 8, 2002.
