Description
NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are b eing requested, and a written solicitation will not be issued. The solicitation number is DAKF06-02-T-0017 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Feder al Acquisition Circular 97-25. This will be a 100% small business set aside. (NAICS: 561499, size standard $5 M). Responses should be quoted in accordance with the following: CLIN 0001, Quantity = 1 year. Period of Performance: 1 March 2002 through 28 February 2003. Paper Shredding/Document Destruction Services (1) Service agreement to provide on-site document destruction, to include 40 secure consoles for the purpose of collecting and securing patient sensitive material prior to shredding. Vendor sha ll collect material from consoles placed at 40 locations throughout six floors of a 400,000 sq. ft. hospital. (2) Material must be kept confidential and secure throughout all phases of the collection and destruction processes. Shredding process shall uti lize, at a minimum, a hammermill or equivalent process to ensure destruction of material, with no opportunity for document reconstruction. (3) Shredding is to be performed on-site weekly and is based on an estimated 5,000 to 10,000 lbs. per week. (4) Mat erials to be destroyed include paper, plastic, radiological films, disks and tapes. (5) Vendor shall provide on-site personnel with no prior criminal record who are bonded, insured, uniformed and badged while on the premises. (6) Vendor shall issue a Cer tificate of Destruction at the end of each service to certify that all material has been destroyed. (7) Shredded material is to be baled and recycled. (8) Document destruction must meet HIPAA and JCAHO standards to certify shredding of Privacy Act materi al in a medical environment. Warranty: Provide with quote standard commercial warranty information. Delivery will be to: Evans Army Community Hospital, Bldg. 7500, Fort Carson, CO 80913. The provision at FAR 52.212-1 Instructions to Offerors ? Commerci al Items and FAR 52.212-4 Contract Terms and Conditions are applicable to this solicitation. Offerors shall include with their offers a completed copy of the provision at FAR 52.212-3 Offeror Representations and Certifications ? Commercial Items.
