- This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation WILL NOT be issued. Solicitation number RFQ-797-FDF4-02-0014 is issued as a Request for Quotation. The solicitation document and incorporated provisions and clauses are those in effect through FAC 01-03. The NAICS code is 421610 and the small business size standard is 100 employees. This action is set aside for small business. The Air Force Medical Logistics Office/Veterans Affairs Special Services (AFMLO/VASS)Contracting Activity has a requirement for thermal walk mats for the medical treatment facility located at Tinker AFB, OK 73145. The contractor shall provide thermal mats that protect walkways leading into the facility. The mats shall be heated insulated mats that protect the surface from ice and snow. Mats must also have a safety feature that will prevent slips and falls. The following specifications, quantity and required dimensions are as follows: Pharmacy entrance (1 each 24' x 28"; 1 each 25' x 28" and 1 each 36' x 28"), Satellite Pharmacy entrance (1 each 31' x 28"; 1 each 35' x 28"; 1 each 35 1/2 x 28"; 1 each 10 1/2' x 28"), Dental North Side entrance (1 each 36' x 28", 1 each 36' x 28" , 1 each 26' x 28"), Bldg 334 entrance (1 each 36' x 28"; 1 each 24' x 28"), Upper Main Door entrance (1 each 26' x 28"; 1 each 33' x 28"; 6 each 35' x 28"); Lower Level entrance (1 each 33' x 28"; 1 each 27' x 28"; 2 each 36' x 28"; 2 each 30' x 28"), Tinker Wellness entrance (1 each 33' x 28"; 1 each 30' x 28"), Triage entrance (1 each 36' x 28", 1 each 27' x 28", 1 each 36' x 28"), Mental Health entrance(1 each 27' x 28", 2 each 20' x 28" 1 each 27' x 28" 1 each 18' x 28" 1 each 15' x 28"); Dental Construction area (1 each 25' x 28"; 1 each 21' x 28", 1 each 13' x 28", 1 each 8' x 28")and Bldg. #1133 entrance(1 each 27' x 28"). Delivery is FOB destination to the 72nd MDSS/SGSL, Tinker AFB, OK 73145 no later than March 13, 2002. The following FAR clauses apply to this acquisition. FAR clause 52.212-1,Instructions to Offerors--Commercial Items; FAR Clause 52.212-4, Contract Terms and Conditions--Commercial Items; FAR Clause 52.212-3, Offeror Representations and Certification-Commercial Items; and FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certification-Commercial Items, with the offer. The Government anticipates award of a purchase order resulting from this solicitation to the responsive/responsible offeror whose offer conforms to the solicitation and is most advantageous to the Government.
