Description
This is a combined synopsis/solicitation for Audiometric equipment and Installation in Okinawa, Japan in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Written quotations are being requested. A written solicitation will not be issued. Solicitation # N62604-02-Q-0978 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-14 and Defense Acquisition Circular 91-13. This acquisition is Set-Aside for Small Business. The standard classification code is 421450; small business size standard is 100 employees. There are Three (3) contract line items. Equipment will be shipped to the Naval Construction Battalion Center, Gulfport, MS. Installation will be at Okinawa Japan. Government requires installer to coordinate delivery, unpack, stage, disburse to rooms per Government P.O.C., and dispose of trash. Keep a list of all items received damaged. Inform CBC Gulfport of all discrepancies IMMEDIATELY.. TO INCLUDE ANY SPECIAL TOOLS OR LIFTS NEEDED TO INSTALL THE EQUIPMENT. SUPERVISOR EXPECTED TO BE REQUIRED NO MORE THAN 2 WEEKS. CONTRACTOR MUST PROVIDE INSTALLATION BREAKDOWN OF COST TO INCLUDE; LABOR RATE, MAN HOURS TO ACCOMPLISH, MEALS, LODGING, AIR FARE, PROFIT AND ANY OTHER RELATED COSTS. THE GOVERNMENT WILL COORDINATE THE INSTALLATION FOR THE TWO CLINICS AND ARRANGE DELIVERY TO THE CLINICS. CONTRACTOR MUST COORDINATE WITH THE POINT OF CONTACT FOR DELIVERY TO JAPAN, OFF LOADING OF MATERIAL, INSTALLATION AND ASSEMBLY IAW MFG?S SPECIFICATIONS, REMOVAL/DISPOSAL OF ALL TRASH & PACKING MATERIALS, FINAL INSPECTION OF PRODUCT. FRIEGHT/TRANSPORTATION CHARGES FOR SHIPPING EQUIPMENT TO GULFPORT NEEDS TO BE SHOWN SEPARATLEY. DRAWINGS AND SPECIFICATIONS FOR ALL EQUIPMENT MUST ACCOMPANY BID PACKAGE.
