Description
This solicitation is issued as a request for quotation (RFQ). The complete solicitation is included as a supplement to this notice; paper copies will not be provided. The solicitation incorporates provisions and clauses that are in effect through Federal Acquisition Circular 2001-03. The acquisition is unrestricted and offers are being requested from firms of all sizes. The NAICS code for the solicitation is 811219; a small business firm is defined as one whose averaged annual receipts for the past three fiscal years is less than $5 Million. The work requires a contractor to provide all labor, tools, parts, materials, equipment, and transportation necessary to perform all operations necessary to repair P/M Calibrate two (2) clinical lasers at the Michael OCallaghan Federal Hospital, Nellis AFB NV. The lasers are (1) Candela Model PLTL-1, S/N 991411304257; and (2) Candela Model SPTL-1B, S/N 990404100206. Offers submitted shall include all costs to fully repair the equipment and shall identify the time required to make the repairs. Award will be made to the responsible, responsive offer who provides the best overall value to the government. Evaluation factors are: (1) Technical capability of the contractor; (2) recent past performance/experience in repairing like items; and (3) price. Technical capability and past performance/experience, when combined, are equally as important when compared to price. Offers are due by 4:30 p.m. PST on 8 Feb 2002 and may be FAXd to 702-652-5405 or mailed to the buyer at the 99th Contracting Office/LGCB, 5865 Swaab Blvd, Bldg 588, Nellis AFB NV 89191-7063. NOTE: Prospective contractors must be registered in the Central Contract Registration (CCR) in order to be eligible for award. Registration requires applicants to have a DUNS number. Contractors may access the CCR at http://ccr2000.com . Recommend registering immediately in order to be eligible for timely award.
