- The U.S. Coast Guard intends to purchase a Personal Wellness Profile (PWP) System. A PWP is an assessment of current health status. It is a program consisting of a lifestyle assessment, a professional consultation and appropriate referral to selected community resources for further evaluation and training in specific lifestyle changes. The following areas are assessed by PWP: ? Physical Activity and Fitness ? Eating Habits ? Heart Health ? Cancer Risk ? Stress and Coping ? Osteoporosis ? Safety ? Preventive Exam The components that make up the PWP are as follows: ? PWP Support Pacs 2000 Ea. (Including Questionnaires) ? APW 138 Scan Module 2 Ea. ? NCS Opscan 2 Optical Mark Scanners 2 Ea. ? PWP Health Guides 5000 Ea. (Sent to 13 locations Hawaii and Alaska listed on attached documents) This notice is a combined synopsis/solicitation for a commercial item in accordance with Federal Acquisition Regulation Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Therefore, please read this entire notice, the instructions herein, and the attachment documents carefully. This acquisition will be made on an Unrestricted Basis: NAICS 621999. See Numbered Note 22: This proposed procurement is for supplies for which the Government intends to solicit and negotiate on a sole source basis with Wellsource, Inc., 15431 SE 82nd Drive, Clackamas, OR, 97015. Interested persons may identify their interest and capabilities with a Notice of Intent. However, this is not a request for competitive quotations. All correspondence must be received by February 21, 2002. A determination by the Government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement. SCOPE: (See attached) HOW TO RESPOND: In order to compete for this project, interested parties must demonstrate that they are qualified to perform the work by providing a quote by 4:00 PM, ET, February 21, 2002. Offerors must submit an original and two copies of the quote to Crystal Kelly, USCG Headquarters Support Command, Code A-3, Room B411, 2100 Second St. SW, Washington, DC 20593. Electronic transmissions to clkelly@comdt.uscg.mil are encouraged and must be timely. Proposal Evaluation Criteria: (1) Price and price-related factors. (2) Technical. (3) Past performance of your organization, including adherence to schedules and budgets, the acceptability of previous products delivered, effectiveness of program management, and the offeror's willingness to cooperate with the customer in both routine matters and when confronted by unexpected difficulties. This will be a Best-Value award determination. Price and Past Performance will be a factor, but technical factors are far more important. The following Federal Acquisition Regulations clauses are attached: FAR 52.212-3, Offeror Representations and Certifications-Commercial Items (Aug 00), 52.212-4 (Aug 00). 52.212-5, Contract Terms and Conditions Required to Implement Statutes of Executive Orders-Commercial Items (Aug 00) is attached. The Department of Transportation (DOT), Short-Term Lending Program (STLP) offers working capital financing in the form of lines of credit to finance accounts receivable for transportation related contracts.
