Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR, Subpart 12.12 as supplemented with additional information included in this notice. This notice constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation (RFQ) number 688-45-02. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular number 97-27. The NAIC code for this solicitation is 541512. The small business size standard is $18 million. This solicitation is a small business set-aside. The contractor shall provide all necessary labor, materials, tools, and equipment to perform MSDS DATABASE SERVICES at the VA Medical Center located in Washington, DC covering the base period March 1, 2002 through February 28, 2003, with four (4), 1-year options. Options will be exercised and task orders will be issued for the performance of maintenance services. The project must be supervised by, and work product approved by, a Certified Industrial Hygienist, listed in the current roster of the American Board of Industrial Hygiene. Personnel performing on-site work must have a Bachelors degree from a college or university. The SCHEDULE OF PRICES FOR THE BASE YEAR AND FOUR OPTION YEARS shall be listed and priced as follows: GROUP I. Survey Entire Facility excluding the Research Facility (Building #4) - On-Site Chemical Inventory. Conduct a wall-to-wall inventory of chemical products at the facility. Minimum inventory data fields are product name, manufacturer, location, Service, MSDS date, and hazard warnings (e.g.; flammable, carcinogen, teratogen, sensitizer, irritant, EPA extremely hazardous, etc.). The on-site survey will be started within 3 weeks of contract signing and will be completed within 6 weeks of contract signing. A data copy of the inventory will be provided to the Medical Center in MS Excel format. Obtain MSDSs. Obtain a current MSDS for every product on the inventory. The VA Medical Center will provide the contractor with a copy of the Medical Center's MSDS archive, however, please note that this archive is incomplete and out of date. Web Hosting of Inventory and MSDS Images. Post and maintain a searchable MSDS inventory and MSDS image files on a secure web site. Image files will be linked to the inventory listing and to the search results listings. All web pages will be viewable and printable with recent versions of standard web accessing software. The inventory will be searchable by product name, manufacturer, and keyword. The web-based inventory and database will be fully populated and operational within 6 weeks of the on-site inventory work. The web-based services must be operational and available 24 hours/day, 7 days/week, 365 days per year. Maintenance of System - Quality Assurance. Instances of confirmed malfunction of the web-based services/per year (excluding routine system maintenance for which prior notification has been forwarded to the VA) will result in a $1000 chargeback to the contract per documented and confirmed instance of lost service exceeding 4/year. A confirmed malfunction is the inability of three VA workstation to access any of the searchable MSDS inventory and MSDS image pages, when the workstations may easily access other non-governmental web sites. Archive Copies of Inventory and MSDSs. Assemble and deliver two hard copies of the MSDS inventory and MSDSs in alphabetically tabbed loose-leaf binders and deliver these within 6 weeks of completion of the on-site inventory work. Quarterly Updating. Update the web-based MSDS database and inventory on a quarterly basis from changes or additions sent to the contractor by the Medical Center. Provide updated inventories and copies of the changed MSDSs in order to update the two archive copies of the MSDSs and inventory. Training. Provide an on-site training session to key staff at the Medical Center to ensure they understand how to access and use the web-based services. Annual Re-Survey. Conduct annual on-site re-survey of the facility, with updates the web-based MSDS database and inventory, and deliver new sets of the archive copies of the inventory and MSDSs to the Medical Center.
