- Description
- The Carl T. Hayden VA Medical Center, 650 E. Indian School Road, Phoenix, AZ 85012 anticipates contracting for prosthetic services for eligible veterans. Services are to be performed at the VA Medical Center. Offers will be accepted only from offerors who are financially responsible. All responsible offerors may submit an offer and will be considered. It is the intention of the VA to award a firm fixed price contract. The contract base year is anticipated from April 1, 2002 to March 31, 2003 with four (4) one (1) year renewal options. This is a total 100% small business set-aide and offerors are reminded to complete all sections of the solicitation, including the Offeror Representations and Certifications - Commercial Items. The NAICSC code is 621399, size standard $5.0 applies. Solicitation documents will be available electronically on or about February 21, 2002 at http://www.va.gov/oa&mm/busopps.htm.. NO TELEPHONE requests shall be accepted. All offerors are reminded to check for possible solicitation amendments electronically at the same web site.
