Description
This is a combined synopsis/solicitation issued for commercial items and is prepared in accordance with the format in Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items, Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, Part 13, Simplified Acquisition Procedures, and as supplemented with additional information included in this notice. This procurement is being conducted under Federal Acquisition Regulation (FAR) Subpart 13.5 test program; simplified acquisition procedures apply and the VA will conduct the procurement according to a combination of Parts 12 and 13. This announcement for Request for Quotation RFQ 618-114-02 entitled ?Lease of Coherent Versapulse Powersuite Holmium Laser System? brand name or equal hereby incorporates provisions and clauses in effect through Federal Acquisition Regulation FAC 2001-04 and VAAR 97-8. This is an unrestricted procurement. NAICS Code 334513 applies for this procurement and the small business size standard is 500 employees. A separate written solicitation document will not be issued. Contractor?s proposals are hereby being requested in response to requirements listed below and shall be submitted on contractor?s letterhead and must address all requirements listed below. Contractor?s response should contain the name, title and signature of person authorized to submit proposals/bids on behalf of the contractor. Descriptive literature for each item offered must also be included in proposal/bid package. The VA Medical Center (VAMC), Minneapolis, MN, has a requirement for a three-year operating lease for brand name or equal Lumenis, Inc., 2400 Condensa Street, Santa Clara, CA 95051-0901 equipment. VA requires a termination option at the end of year one and year two. At the conclusion of the three-year lease term, VA requests an option to purchase at fair market value the equipment or return the equipment to contractor at no additional cost. Brand name or equal equipment is described as follows: Lumenis, Inc., 2400 Condensa Street, Santa Clara, CA 95051-0901 equipment: (1ea) Coherent Versapulse Powersuite? 60 Watt Holmium Laser system configured with the following (a) Pulsed holmium laser at 2,100 nm at an operation range of 5 to 40 Hz and 0.2-3.5 joules. (b) Fiberoptic port that accepts flexible fiber delivery systems designed for arthroscopy, urology, lithotripsy, general surgery, discectomy, ENT and other surgical procedures. (c) 2.5 mw red diode aiming beam with adjustable intensity. (d) Color Touch screen control panel allows for simple adjustment of all settings. (e) Inspection Microscope for inspection of re-usable fiber delivery systems. (f) Foot Pedal. (g) Remote Control. (h) 2 Debris Shields. (i) 3 pair of safety glasses and 2 pair safety goggles. (j) Operator manual, 2 laser safety signs, accessory bag. (k) In-hospital laser training program for the O.R. nursing staff by a Lumenis, Inc. Regional Education Manager (qualifies for up to 4.0 contact hours by ANA). (l) Warranty on parts, labor ? twelve months; Y2K Compliance Warranty. Delivery terms shall be FOB Destination (unless vendor states otherwise) to the VA Medical Center, Warehouse (90D), One Veterans Drive, Minneapolis, MN 55417. This will be a firm fixed price lease agreement. Payment may be made by Government Purchase Card (VISA). Evaluations may be conducted to determine that the products offered will meet Government?s requirements. Offerors must indicate in their proposals the price at which they would be willing to sell the equipment should the Government elect to purchase at a later date, prior to expiration of the lease agreement; offerors must also specify the latest date such an option may be exercised by the Government and how much of the lease cost already paid by the Government may be applied against the purchase price quoted. Evaluation factors of technical capability of the items offered and past performance history, and delivery time frame will be considered along with the maximum period of use at the lowest total price to determine ?best value? to the Government. Evaluation may be made with or without discussions with the offeror(s). All responsible sources that can meet the requirements and provide items as listed above may respond to this solicitation by submitting cost and technical proposals on their company letterhead or bid form.
