- Statement Of Work Obstetrical Services for the Fort Peck Service Unit in the Billings Area Indian Health Service I. Acquisition Background and Objectives 1. Introduction The mission of the Fort Peck Indian Health Center is to provide the best possible health care services to the Native American population on the Fort Peck Indian Reservation in Montana. To meet the mission, the Fort Peck Service Unit requires contractor support to assist the service unit in the recruitment and placement of obstetrical provider(s) into a temporary duty assignment in the Fort Peck IHS Health Center. The contract effort shall be tailored to provide a wide range of obstetrical services. 2. Background Information The Fort Peck Service Unit primarily serves two tribes, the Assiniboine and the Sioux tribal nations. The user population is approximately 8,500 members, and the Service Unit facility is located in the northeast corner of the State of Montana. There are two ambulatory health care centers located on/around the reservation, one located in the township of Wolf Point (Chief Redstone) and the other one in the township of Poplar (Verne E. Gibbs). The majority of the approximately 170 deliveries per year take place at the Trinity Hospital, a private hospital located in the Wolf Point community. 3. Purpose of the Contract The purpose of the contract is for the recruitment and placement of provider(s) of obstetrical services into the Fort Peck Indian Health Service facility. The care provided by the Contractor, shall be as comprehensive as the Indian Health Service supplied facilities, equipment and support services permit. The Contractor shall furnish the necessary personnel, to provide obstetrical services to the Fort Peck Service Unit. The Indian Health Service requires that obstetrical services continue without interruption during the term of this contract. 4. Scope of Work The Fort Peck Service Unit proposes to contract all of its prenatal, intrapartum, postpartum and newborn care to qualified provider(s) through the award of a Non-Personal Health Care Contract. The Fort Peck Service Unit reserves the right to select more than one successful contractor if the Fort Peck Service Unit?s population needing prenatal, intrapartum, postpartum, and new born care can be served most efficiently through more than one contract. The contractor shall furnish obstetrical provider(s) for primary care services at the Fort Peck Indian Health Center. The contractor shall perform services that are compatible with the Fort Peck Indian Health Service medical facilities operating equipment. The contractor shall provide obstetrical services for Indian Health Service beneficiaries served by the Fort Peck Service Unit. The contractor?s services shall cover the range of obstetrical services typically found in a rural civilian practice of similar size. Contract performance shall be according to the requirements contained in this Statement of Work. The standards of medical practice and duties of the contractor through its obstetrical services shall be determined pursuant to the by-laws of the Fort Peck IHS Health Center, the regulations of the hospital/health center, applicable provisions of law, other rules and regulations of any and all governmental authorities relating to licensure and regulation of providers and hospitals and the standards and recommendations of the American Academy of Ambulatory Health Care (AAAHC). The contractor shall be required to participate in a quality improvement program, which is consistent with current AAAHC requirements for ongoing monitoring and evaluation of the quality and appropriateness of care. A contractor shall not perform medical services until professional staff membership and the Indian Health Service governing body has granted clinical privileges in writing. As a prerequisite to performance under the resultant contract, the contractor performing these services must be privileged in accordance with Indian Health Service/Billings Area credentialing policy. Therefore compliance with the Indian Health Service privileging requirements is essential in the performance of the contract. Any failure to comply with the IHS privileging requirements shall be considered to be in non-performance of the contract. The governing body will not grant clinical privileges until it is determined that credentials information has been verified. Exceptions to the Indian Health Service credentialing review and clinical privileges process will not be considered. Any contract provider who fails to maintain medical staff appointment and clinical privileges will not be permitted to perform services under the contract and shall be promptly replaced by the contractor. The contractor shall locate and have available sufficient staffing to provide obstetrical services on a regular schedule to the Fort Peck Service Unit. The work shifts shall be established at the time of contract award and may be adjusted when mutually agreed upon. Work shifts will normally be eight (8) to ten (10) hours in length and will be worked between the hours of 6:00 am and 6:00 pm at 4 days per week and will provide 1 specialty clinic every two weeks. Contract performance will generally not be required on Federal holidays. Prior to assumption of duty on any scheduled work shift, the contract obstetrical provider may be briefed by the service unit chief medical officer or his designated representative concerning any matter relevant to the contractor?s assumption and discharge of responsibilities under the terms of this contract. The contractor shall advise the chief medical officer or his designated representative of any problem encountered or which may be encountered in connection with meeting the needs of patients treated by the contractor. The briefing shall also include conditions of any inpatients for whom the contractor may reasonably be expected to attend during the work shift. The contractor shall report continuing problems of any nature to the designated Indian Health Service chief medical officer. The contractor shall generate and maintain proper medical record information on beneficiaries to whom treatment is provided in accordance with Indian Health Service regulations. The contractor shall prepare all medical record documentation to meet or exceed the established standards of the IHS facility to include but not be limited to: timeliness, legibility, accuracy, content and signature. All medical record information generated in the performance of this contract will remain the property of and subject to the exclusive control of the Indian Health Service. Treatment of IHS patients shall be performed at the IHS facilities except in certain instances when patients are referred to private facilities for performance of special diagnostic procedures or definitive care not offered in the IHS facility. All referrals to private facilities or providers for specialized diagnostic studies or definitive care not offered at the Fort Peck IHS facility shall follow Indian Health Service regulations and policies on patient referrals. The contractor shall become familiar with and be guided by Indian Health Service formulary. The contractor shall follow Indian Health Service procedures when prescribing drugs. The contractor authorized to prescribe pharmaceuticals shall do so according to the availability of drugs listed in the IHS formulary. IHS pharmacy services will provide instructions to the prescribing contract practioner as to substations of generic drugs for prescribed drugs. Drugs not on the formulary must be approved by the service unit pharmacy and therapeutics committee. The contractor shall assess obstetrical services being performed to assure its quality. The contractor shall participate in a continual process of self-evaluation and strive for excellence in providing obstetrical services. The contractor shall meet all applicable AAAHC standards during the performance of this contract. The contractor shall respect and maintain the basic rights of patients, to include but not be limited to personal dignity. 5. Applicable Documents ? Technical Requirements Contractor must provide a documented, clearly defined path of referral for hospital care of the normal obstetrics patients as well as for the woman who develops complications during pregnancy, labor and/or delivery. Contractor shall provide a documented plan of care for deliveries at a designated healthcare facility that has adequate personnel in nursing, anesthesia, surgical, obstetrical and pediatric providers. Services such as anesthesia, obstetrics, pediatric coverage and competent obstetrical nursing staff must be continuously available. Failure of the designated facility to meet these standards can result in cancellation of this contract. After delivery, the designated hospital facility must provide timely records of care for both the mother and the newborn to the Fort Peck Service Unit. The contract resulting from this solicitation shall: 1. Be a non-personal health care services contract under which the contractor is an independent contractor. 2. Allow the Government to evaluate the quality of professional and administrative services provided, but retains no control over the medical, professional aspects of the services rendered, (e.g., professional judgments, diagnosis for specific medical treatment); 3. Require the Contractor to indemnify the Government for any liability producing act or omission by the Contractor, its employees and agents occurring during contract performance; 4. Require the contractor to maintain medical liability insurance, in the coverage amount acceptable to the Contracting Officer, which is not less than the amount normally prevailing within the local community for the medical specialty concerned; and 5. Require the contractor to ensure its subcontracts for provisions of health care services, contain the requirements of the clause at FAR 52.237-7, including the maintenance of medical liability insurance. Government Furnished Property, Facilities and Services The service unit shall provide office space including a desk, chair and office supplies. The contractor will be authorized to use all areas of the medical treatment facility available to civil service and commissioned personnel in similar positions. The same restrictions apply to the contractors as any other Indian Health service personnel to use these areas for official business activities only. The contractor will be authorized to use all administrative and professional support available to Indian Health Service personnel. This will include but not be limited to copy machines, facsimile machines, and medical library and telephone services. The same restrictions to use these items for official Indian Health Service business apply. The contractor shall be provided with an orientation to the Indian Health Service and the Indian community being served. Service unit personnel shall provide the initial orientation and continuing orientation. The service unit shall provide all medical and non-medical equipment and supplies used in the Fort Peck Indian Health Service facility for the care and management of patients. The service unit shall provide all required blank Government forms applicable to the Indian Health Service. Service unit personnel will provide an appropriate orientation to the necessity of such forms. The service unit shall provide an ongoing method of monitoring and evaluation of the quality and appropriateness of patient care and of identifying deficiencies in the quality of services performed. Generally, the service units will not provide patient transcription services. Legible handwritten documentation of medical records is acceptable in place of recorded dictation. Contractor Furnished Equipment, Facilities and Services The Indian Health Service does not anticipate a need for contractor furnished equipment. 2. Deliverables The Service Unit proposes to contract all of its prenatal, intrapartum, postpartum and newborn care to a qualified provider through the award of a Non-Personal Health Care Contract. The Service Unit reserves the option of selecting more than one successful contractor if the user population needing prenatal, intrapartum, postpartum, and new born care can be served most efficiently through more than one contract. Should more than one contract be awarded to provide care for our population, the overall needs of the Service Unit?s population must be met in entirety through the combined deliverables of the multiple contracts. Obstetrical Provider Responsibilities The contractor shall provide a board certified or board eligible physician trained in obstetrics and gynecology to provide physician services for patients who require a category of obstetrics and/or care for complex or severe illness or problems and those with immediate or serious threat to life. The contractor shall meet the standards established by the American Board of Obstetrics and Gynecology for board certification or board eligibility. The contractor shall provide obstetrical service delivery staff to include outpatient services, inpatient services, antepartum care, labor and delivery, emergency care, surgery and postpartum care. The contractor shall perform examination and treatment of authorized Indian Health Service beneficiaries according to the daily clinic schedule or as required by the service unit chief medical officer. The contracted staff shall possess the skills; knowledge and training satisfactorily perform the services required by this contract. All contractors utilized under this contract must be able to obtain clinical privileges to practice obstetrics and gynecology at the service unit. The contractor shall be under the technical supervision of the service unit director and chief medical officer. The contractor shall be knowledgeable in all areas that are, as a standard, covered by an accredited obstetrical residency. The contract staff shall have sufficient experience to adequately diagnose and treat a full range of obstetrical and gynecological cases, procedures and surgeries. Obstetrical Physician Qualifications The contracted staff shall be board certified or board eligible without additional education or training, by the American College of Obstetrics and Gynecology. It is a condition of this contract that all required certifications in this Statement of Work remain current during the life of this contract. The contracted staff must hold a current, unrestricted state medical license from any of the 50 United States or the Commonwealth of Puerto Rico. The contracted staff shall meet the same privileging requirements as permanent IHS physicians before being allowed to provide medical services at the Fort Peck IHS facility. The contracted staff shall not have any work or health restrictions, which interfere with the performance of providing services. The contracted staff shall possess sufficient initiative, interpersonal relationship skills and social sensitivity such that he/she can relate constructively to patients from the Native American community. The contractor shall maintain standards of personal hygiene and grooming such that his/her image as a health care provider is compatible with the expectations of the Fort Peck Indian Health Service Health Center clinical staff and the Indian community.
