Description

This is a combined synopsis/solicitation for Commercial items prepared in accordance with the format in FAR subpart 12.6 and supplemented with additional information included in this notice. This announcement constitutes the solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number is F61040-02-Q0001. The solicitation is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-04 and DFARS Change Notice 2002-0002. All responsible sources shall submit a written proposal, which will be considered. Facsimile submissions will also be accepted. The North American Industry Classification System (NAICS) code for the solicitation is 811219; the small business size standard is $6,000,000.00 (USD). This combined synopsis/solicitation is unrestricted. Item/Services Description: Non-Personal Service: Contractor shall furnish all labor, transportation and per diem to repair one General Electric Fluoroscopic and X-Ray Portable System at 65 MEDG, Lajes Field, Azores, Portugal. Manufacturer: General Electric, Model No:AMX-4, Serial No: 10504 The Azores is located in the mid-Atlantic about 2,300 miles east of New York and 900 miles west of Lisbon, Portugal. Lajes Field is located adjacent to the airport, on the NE corner of the island (close to Praia da Vitoria). Local Airport designation is TER. There are local hotels on island; using internet search engines (Keywords Lodging, Terceira, Azores). Date of Delivery: Contractor is not to repair the system until notified by the Contracting Officer or his representative that the parts have arrived. Parts are provided by the 65th MEDG. Delivery Point: 65th MEDG/Laboratory, APO AE 09720 PROPOSAL PREPARATION INSTRUCTIONS: The provision at 52.212-1, Instructions to Offerors - Commercial items, applies to this acquisition. Offerors shall provide a firm fixed-price offer. Offerors shall provide current certification or other documentation reflecting the capability of the individual who will be performing the repair on this General Electric Fluoroscopic and X-Ray Portable System. The Government will award a contract resulting from this solicitation to the responsible offer whose offer conforming to the solicitation will be most advantageous to the Government, considering price and technical capability. Offerors who cannot provide documentation reflecting the technical capability to perform repairs on the stated equipment will not be considered for award. Offerors shall include a completed copy of the provision at 52.212-3 Offeror Representations and Certifications?Commercial Items with their offer. Successful contractor must meet all the responsibility requirements of Subpart 9.1 of the Federal Acquisition Regulation (FAR). A written notice of award or acceptance of offer, mailed or otherwise furnished to the successful offeror, shall result in a binding contract without further action by either party.
