- Description
- This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-02 -T-0025 is issued as a request for quotations(RFQ). Clin 0001: Non-personal service contract for a Research Coordinator for the period 14 Apr 2002 to 13 Apr 2003. Qty: 2080; Unit of issue: Hours. The duties to be performed shall be in accordance with the statement of work. The statement of work reads: The Research Coordinator serves as the principal office and research assistant, performing administrative, data collection and analysis duties in support of ongoing and future refractive surgery research projects. The research coordinator is the primary liaison between internal or external work units. The incumbent provides administrative and research assistance to the Chairman, Ophthalmology Department, the Refractive Surgery Program Director, Research Director, Clinic Director, and Division Officer. The incumbent is directly responsible to the Refractive Surgery Research Director. 2. DUTY HOURS: a. The contractor shall be available for duty at the Naval Medical Center, San Diego or the Naval Training Center, San Diego, CA for a total of eight hours between the times of 0630-1700 (this includes a one-hour lunch), Monday through Friday. The contractor shall agree to extend these hours, as necessary, to ensure experiments are completed or deadlines met. The contractor shall be compensated for these extended hours on a non monetary basis, time off duty of match additional time on duty at the discretion of the Department Head. The contractor shall arrive for each scheduled shift in a well-rested condition and shall have had a least six hours of rest from all other duties immediately prior to reporting for shift. 3. DUTIES: Organizes research charts for consent. Ensures all necessary information is contained and signed (if need be) for subject surgery and study subjects consent with provider. Notifies appropriate points of contact if chart is incomplete. Ensures that subjects have signed consent prior to surgery and that provider has signed as well. The incumbent will compile research results from subject measurement into appropriate databases, and participate in the analysis of the data. Assist in general data entry of subject records into a computerized database. Assist in the preparation of tables and graphs for manuscripts and presentations of research analysis/data. Assist in determining subjects eligibility for research studies in refractive eye surgery, and contact subjects accordingly. Coordinates research schedule calendar for the Program Director, Research Director and other clinical investigators. Liaisons subject examination for study subjects. Makes arrangements for time, participants and locations of meetings in accordance with instructions from the Program and Research Directors. Controls access to and manages the schedule for the Clinic Conference Room ensuring all conferences, classes, pharmaceutical representatives, and other commands onboard Branch Medical Clinic have access for required meetings, presentations and training. Screens all incoming calls and visitors for the Research Division. Uses knowledge of department organizations, functions and policies to determine priority of the call or visit and furnishes information personally or refers callers to the appropriate points of contact. Takes and transcribes minutes for weekly and monthly staff meetings. Maintains an action item tickler to keep track of action due dates set for completion agreed upon at meetings. Serves a Liaison for the Ophthalmology Department/PRK Program with other commands, departments and services. Sorts, processes and prioritizes incoming and outgoing research correspondence except for mail addressed personally to a supervisor or ?Eyes Only?. Retains correspondence that can be handled personally and composes routine replies. Uses the Composite Health Care System (CHCS) to schedule subject appointment, and Electronic(Email) system to reply to messages to personnel in the clinic, subjects, and to other commands or organizations. Establishes and maintains appropriate files; maintains correspondence tracking log and tickler file for reports. Responds to routine and non-technical requests for information readily available from files. This may involve determining whether or not to release the information based on the nature or degree of sensitivity of the information and the requester?s authority to receive information. The incumbent uses a variety of office automation software in support of the organization. Determines the best software type of each report (such as database or spreadsheets). Uses graphics software to provide graphics symbol, charts, and graphs for viewgraphs or paper presentations. Accurately and promptly transmits and receives documents and messages electronically using PC?s or workstations. Utilizes Microsoft Office computer programs to prepare reports and correspondence from original of draft formants to include ?By direction? items if appropriate and makes revisions as required in accordance with the DOD Correspondence Manual. Prepares correspondence, reports, and other research documents: Accurately prepares correspondence, reports, or similar materials from handwritten or electronic drafts. Ensures correct grammar, spelling, capitalization, punctuation and appropriate format in work prepared by self and others. Returns material needing correction to the originator when necessary. Updates research clinic phone listings and the providers recall bill to include pager number and other pertinent information. Assists the reception and scheduling staff with checking in subjects, scheduling subject appointments as it applies to research protocols. Inform subjects of research study schedule and appointments. Prepares Subject Information Packets and mails to non-local subjects. Maintains packets returned with proper signatures, and notifies subjects if incomplete/not received. Notify and inform subject Commanding Officer about subject participation in a research project. Inform Commanding Officer about study requirements and time away from parent command for study participation. Ensure CO approval is obtained prior to study enrollment. 4. REQUIREMENTS: An Associate?s/B.A/B.S degree in science with relevant laboratory experience or equivalent education and work experience preferable, but not required. Ability to communicate orally and in writing using grammatically correct English. Knowledge of grammar, spelling, capitalization, and punctuation to process a variety of correspondence accurately. Basic knowledge of Navy and Marine Corps rank and military customs and courtesy preferable but not required. Knowledge of Navy Correspondence formats, forms, command correspondence policies and military (Navy/Marine) terminology preferable, but not required. Knowledge of the duties, priorities, commitments, policies and program goals of the staff sufficient to perform assignments such as reviewing publications, directives, and other materials which may effect the Clinic. Knowledge of standard office equipment such as typewriter, copiers, fax machines, etc. to perform routine clerical and administrative functions. Knowledge of PCs and software such as word processors, graphics, spreadsheets, database management systems and communications; specific familiarity with MS Word, Excel, Power point and Outlook. Advanced skills in computer applications with the ability to operate and input data to obtain reports, forms, compose correspondence, charts, graphs, messages, letters, etc. in Microsoft Office, Word, Power Point, Excel, Access, Outlook and various other programs developed for the Navy?s use. Knowledge of Privacy Act regulations pertaining to the confidentiality of Health records, medical information contained therein, and the security requirements of such documents and information. Ability to compose technical and non-technical correspondence from handwritten notes, recordings, etc. Knowledge of guidelines which include: dictionaries, Navy Correspondence Manual, Clinic and departmental SOPs, Department of the Navy and Department of Defense Instructions and Manuals, concerning such matters as correspondence procedures of time and leave regulations, and the operating policies of the supervisor. Judgment should be used in locating and selecting the appropriate guidelines, references and procedures for application to specific cases, referring significant deviations or situation to which existing guidelines cannot be applied to the supervisor. Incumbent interprets, applies and/or adapts these guidelines in performing major duties. The contractor shall comply with the medical staff rules, regulation and bylaws related to clinical practice as promulgated by the Medial Treatment Facility Commander and the nationwide standards of practice of the Ophthalmology specialty. The contractor will be neat, clean, will groomed and in appropriated clothing when in patient care areas. Hair shall be neatly trimmed and combed. The contractor shall display an identification badge on the right breast of his or her outer clothing, which includes the contractor?s full name and professional status. The contractor shall become acquainted with and obey all station regulations, shall perform in a manner to preclude the waste of utilities, and shall not use Government telephones for personal business. All motor vehicles operated on the installation by the contractor shall be registered with the base security service according to applicable directives. Eating and smoking by the contractor are prohibited in patient care areas and are restricted to designated areas. The contractor is not prohibited, by reason of his or her performance under this contract, form outside employment so long as there is no conflict with the performance of services under this contract. The contractor shall make no use of any Government facilities or other Government property in connection with outside employment. All financial, statistical, personnel, and technical data which is furnished, produced or otherwise available to the contractor during the performance of the contract are considered confidential business information and shall not be used for proposed other than performance of work under this contract nor be released by the contractor without prior written consent of the Department Head or designated representative. Any presentation of any statistical or analytical materials, ore any reports based on information obtained from studies covered by this, will be subject to review and approval by the Department Head or designated representative before publication or dissemination. The Secretary of the Navy has determined that the illegal possession or use of drugs and paraphernalia in a military setting contributes directly to military drug abuse and undermines command efforts to eliminate drug abuse among military personnel. The policy of the Department of the Navy (including the Marine Corps) is to deter and detect drug offenses on military installations. Measures taken to identify drug offenses on military installations and to prevent introduction of illegal drugs and paraphernalia include routine random inspection of personal possessions on entry or exit. If there is probable cause to believe that the contractor has been engaged in use, possession, or trafficking of drugs, the contractor may be detained for a limited period of time until he or she can be removed from the installation or turned over to the local law enforcement personnel having jurisdiction. When illegal drugs are discovered tin the course of an inspection or search or a vehicle operated by a contractor, the contractor and vehicle may be detained for a reasonable period of time necessary to surrender the individual and vehicle to appropriate civil law enforcement personnel. Action may be taken to suspend working privileges as well as installation driving privileges. Implicit with the acceptance of this contract is the agreement by the contractor to comply with all Federal and State laws as well as regulation issued by the commander of the military installation concerning illegal drugs and paraphernalia. 5. FAILURE TO PERFORM: a. Should the contractor be unable to perform duties under this contract due to medical or physical disability for more than 13 consecutive days or because of questions concerning competency, ethics, or conduct, performance under this contract may be suspended by the contracting officer until such medical or physical disability is resolved, or until such questions concerning competency, ethics, or conduct have been resolved. If performance under the contract is so suspended, no compensation or reimbursement shall accrue to the contractor so long as performance is suspended. The solicitation document and incorporated provisions and clauses are those in effect through FAC 2001-04 . The following clauses and provisions are applicable and will be incorporated into the solicitation by reference, and will be available in full text upon request. The following factors shall be used to evaluate offers: 1) Past performance 2) personnel qualifications based on SOW 3) Price. Past performance and personnel qualifications based on SOW when combined have more importance than cost or price. Prior to Contract Award, the government reserves the right to review the resumes of contractor employees performing under the contract solely for the purpose of ascertaining their qualifications relative to the personnel qualifications terms of the contract. Accordingly, the contractor shall furnish such resumes and all professional requirements as stated in the Statement of Work ( SOW) to the Purchasing Agent along with their quotes.
