Description
NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are requ ested and a written solicitation will not be issued. DAHA17-02-T-0017 is hereby issued as a Request for Quotations. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001. The Nort h American Industrial Classification Code (NAICS) IS 812213 and the business size is 6.0 million. This is a 100% small business set-aside. An award will be made based on technical and price related factors. The following service is required: Maintenance Agreement for Private Branch Exchange Lucent Definity G3siV8 PBX for period 1 May 2002 - 30 April 2003. Vendor must provide maintenance services during the core hours of 6:00 Am to 5:00 Pm Monday through Sunday excluding holidays. 1 hour remote response time for major corrective maintenance. 4 hour on-site response/repair for corrective maintenance. 8 hour on-site response for minor corrective maintenance. Maintenance shall include all service, parts, labor and materials. Contractor has to have the ability to offer remote diagnostics. Contractor has to have the ability to offer remote repair Contractor has to have the ability to automatically dispatch a technitian. Major corrective maintenace occurs during any of the following: 1. Any attendant console cannot place or receive calls. 2. A minium of 10 percent of all telephone and data ports cannot place or receive calls. 3. A minimum of 10 percent of all trunks are inoperative 4. A T1/D3 span is inoperative 5. A minimum of 10 percent of the trunks or agents supported by centralized attendant service or automatic call distribution applications are inoperative or cannot place or receive calls Minor corrective action is any routine maintenance service requested for the PBX not included in the major corrective actions. The PBX is located at Camp Keyes, Augusta Maine 04333. Evaluation of quotes will be in accordance with FAR Clause 52.212-3, Evaluation-Commercial Items, based on technical capabilities and price. Offerers shall include at least 2 business references that th ey supply maintenance to. These references may be checked to evaluate the technical responsibility of the offerer. Quotes shall include a completed copy of FAR Clause 52.212-3, Offeror Representations and Certifications, Commercial Items. Additional app licable FAR Clauses, 52.212 -4, Contract Terms and Conditions-Commercial Items, 52.212-5, Contract terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items.
