Performance period is for twelve (12) months plus two (2) additional twelve (12) month option periods. Delivery shall be FOB Destination to Denver, Colorado. Written requests for the RFQ must be submitted either by U.S. mail, e-mail or fax at (303) 236-2710 and must contain the RFQ number (02CRQQ0092), the requester?s name, complete street address, telephone and fax number.
