Description: NA Personal services contract for Otolaryngology Services at Ireland Army Community Hospital, Fort Knox, Kentucky, during the period 1 June 2002 through 31 May 2003 (with four one-year option periods). This acquisition is unrestricted and all responsible so urces may submit an offer which will be considered. Requests for bid packages must be submitted in writing and may be faxed to 502-624-5869 or 502-624-7165. Telephone requests will not be accepted. Bid packages will not be faxed. Faxed bids will not be accepted. The solicitation will be issued on or about 16 April 2002 with bids due on or about 9 May 2002.
