- The Space and Naval Warfare Systems Center, Charleston, SC is issuing a combined synopsis/solicitation for commercial services prepared in accordance with FAR 13 and the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, proposals are hereby requested. A written solicitation will not be issued. This is issued as a Request for Proposal (RFP), Number N65236-01-R-O048 which incorporates provisions and clauses in effect through Federal Acquisition Circular 2001-04. This requirement will be 100% Small Business Set Aside. The North American Industry Classification System (NAICS) will be 62419, size standard $5 million. The period of performance will be for a 12-month base period and four one-year options. Please provide prices for the following CLINs: CLIN 0001, 1 Lot, Base Year/; CLIN 0002, 1 Lot, Option Year I; CLIN 0003, Option Year II; CLIN 0004, 1 Lot, Option Year III; and CLIN 0005 1 Lot, Option Year IV; STATEMENT OF WORK CIVILIAN EMPLOYEE ASSISTANCE PROGRAM 1.0 INTRODUCTION. The Civilian Employee Assistance Program (CEAP) is a federal government program designed to provide identification, preventive counseling and referral to local community agencies for treatment, rehabilitation, or other assistance for Federal employees/family members with alcohol, drug, or other personal problems. Family member is interpreted to mean immediate dependents of employees, i.e. spouse, dependent children, or other immediate family member who qualifies as a dependent for Federal/State income tax purposes. The CEAP will be provided to the Space and Naval Warfare Systems Center (SPAWARSYSCEN) Charleston employees. The program will include all subdivisions/serviced activities, which consists of approximately 2,250 employees, 33 CONUS (2,220 employees) and 6 OCONUS (30 employees) locations. 2.0 SCOPE. The specific services to be provided include intake counseling, referral, follow-up, and crisis counseling on virtually any kind of personal problem which affects on the job performance or conduct. Services will be provided either in person or by toll free telephone at the employee/dependent's option for all participants. Employees and their dependents are covered under this program during duty and off-duty hours, as long as the employee is employed by SPAWARSYSCEN Charleston or one of its subdivisions/serviced activities. 2.1 LABOR CATEGORIES. The Contractor shall have licensed professional diagnostic and referral counselors available in Charleston SC and all outlying areas where subdivisions/serviced activities are located. At a minimum, the Contractor must have all of the following available on its staff to perform intake/triage functions: a. Licensed clinical psychologist with background in substance abuse. b. Licensed clinical social worker with background in substance abuse. c. Licensed marriage, family, child counselor. d. Licensed physician in psychiatry, board eligible. 2. 2 LICENSE REQUIREMENT. In addition, the Contractor must be licensed properly as required by the State of South Carolina and all other states included in the SPAWARSYSCEN areas of responsibility described in paragraph 1.0. 3.0 APPLICABLE DOCUMENTS. Alcohol and Drug Abuse Programs, Office of Civilian Personnel Management Instruction 12792.1, Department of the Navy Civilian Employee Assistance Program. 4.0 TECHNICAL REQUIREMENTS 4.1 INTAKE COUNSELING. Upon receiving a request for assistance, the Contractor shall conduct intake counseling as follows: a. Determine that the problem presented by the employee or dependent falls within the scope of the program; b. A qualified counselor shall interview the participant; c. Screening and diagnosis of participant's problem(s) and assistance requirements; d. Evaluation of the participant and consultation, if necessary, with a specialist in the problem area (if necessary, medical evaluation and testing to be included); e. Obtaining employee authorization to release information in the case of supervisory referral; f. Providing feedba ck on the employee/participant to the referring supervisor; g. Creation/maintenance of confidential patient records; and h. Explaining the need for and recommending, if necessary, specialized assistance. Intake counseling shall consist of as many sessions as are determined necessary by the Contractor to accomplish the above without cost to the employee. It is anticipated that only in rare cases will more than three counseling sessions per problem be required. 4.2 REFERRAL. When intake counseling has resulted in a determination that treatment or rehabilitation services are required, the Contractor shall refer the participant to appropriate resources which will provide services on a fee basis. The fees will be established between the participant and the service and will be paid by the employee. If a no-fee or low-fee service is available with no significant compromise in quality, referral to this service will be attempted first. This includes service available through/covered under the employee's health benefits plan. The Contractor will maintain current information on health benefits available for federal employees. 4.2.1 FOLLOW-UP. If referral is made, the Contractor shall follow up with the employee to ensure quality of treatment by the agency referred to and to ascertain status of problem resolution. The Contractor shall also follow up to ascertain if the client participated in treatment as recommended. In the case of supervisory referrals and given the employee has agreed, the Contractor shall provide the supervisor with information on the employee's attendance and the prognosis. 4.3 USE OF THE PROGRAM. Annually within the first sixty days of the contact, the Contractor shall provide to the CEAP Coordinator, a ?CEAP Supervisory Training? course in Microsoft PowerPoint. This course shall be designated to familiarize supervisors with the program and to instruct them in proper procedures. Also a CEAP Employee Familiarization course in Microsoft PowerPoint shall be provided by the Contractor to the CEAP Coordinator. The course shall be designed to familiarize all employees with the CEAP and methods to seek assistance/counseling. SPAWARSYSCEN Charleston will provide the courses to employees via computer-assisted training, compact disk, videotape, or other means. The contractor shall also provide information regarding services provided to supervisors and employees in response to telephone, or personal inquiries. All course material is subject to acceptance upon receipt. The contractor is responsible for providing any required changes to the course material throughout the life of the term of the contract. 4.4 PRINTED MATERIAL. Fifty (50) posters for Official Bulletin Boards will be provided within thirty days of the contract award date. The posters shall contain, as a minimum, information about CEAP, the availability of CEAP counseling, and the name, address, and telephone number of the Contractor. The contractor shall provide written information in the form of existing off the shelf brochures explaining CEAP for all current employees, with additional pamphlets available for new hires. A sufficient number of the brochures shall be provided within thirty days of the contract award and annually thereafter to the address shown in paragraph 4.5 to endure each employee will receive one. A copy of the Poster and brochure shall be submitted to SPAWAR System Center Charleston within 15 days after contract award for approval. Contractor shall be responsible for the posters and brochures, as changes are required. 4.5 LOCATION ADDRESS. SPAWARSYSCEN Charleston P. O. Box 190022 North Charleston, SC 29419-9022. 5.0 REPORTS; DATA DELIVERABLES 5.1 STATISTICAL REPORTS. Quarterly and annual statistical reports of program activity will be provided in written form. The data required will be provided to contractor, via his agent, by the activity CEAP Coordinator and shall contain data necessary to complete Federal Employee Assistance Program Report (Worksheet, Civilian Employee Assistance Programs, Semi-annual and Annual Report), or any successor reports. The Contractor shall monitor the total staff years expended in Program Management and Counseling Services; i.e., actual number of full-time and part-time Administrators and Counselors working with the contract during the year. The CEAP coordinator reserves the right to request the information in a different format or request additional information. (See OCPMINST 12792.1 for additional formatted reports.) 5.2 ADDITIONAL DATA. The Contractor shall furnish additional data required for any annual or one time report, even if such data is requested after the expiration of the contract. Data shall be provided in writing within five calendar days after request by SPAWARSYSCEN Charleston. The contractor shall also respond to telephone requests for data. 6.0 SPECIAL CONSIDERATIONS. 6.1 CONFIDENTIALITY. Access to the Contractor for counseling is strictly a confidential matter; prior to knowledge or approval of a supervisor, CEAP Coordinator, Contracting Officers Representative (COR), the Human Resources Office, or anyone else, is not necessary. 6.2 REQUEST FOR ASSISTANCE. Service shall be made available by toll free telephone call seven days a week, twenty-four hours a day. A personal assessment meeting will be scheduled at a time convenient to the employee requesting the assistance. Requests for assistance shall be accepted directly from civilian employees and their immediate dependents on their own behalf. Supervisors may obtain advice and guidance from program practitioners. If an employee is referred to CEAP by a supervisor, the employee's participation is strictly voluntary. In the event a counselor is not available to speak with a participant who calls for assistance, a counselor shall return the call within 48 hours. When a participant is involved in a crisis situation, which is defined as any situation perceived by the participant as a crisis, the Contractor shall provide or arrange for immediate assistance. When a group of employees are involved in a crisis situation, the contractor shall immediately provide or arrange group counseling. Callers will be required to identify the organization to which they, or the civilian employee, on whom they are dependent, are assigned in order to verify eligibility for assistance. The CEAP Coordinator will periodically provide an alphabetical listing of eligible employees to the Contractor for use in verification of employee eligibility for the use of the program. 6.3 CONFIDENTIALITY OF SPECIFIC RECORDS/DATA. The need for strict confidentiality in a diagnostic/referral-counseling program is a prime reason for contracting with a private, independent organization. The Contractor/counselor's records, or any other information pertaining to identity, diagnosis, or treatment of a specific participant, shall be disclosed to SPAWARSYSCEN or others only on the written and signed consent of the counselee. Such consent shall specify to whom the disclosure shall be made, and shall contain a stipulated period of time for which the release is to remain in effect. As stated above, the Government's access to Contractor/counselor records, except with the written consent of the counselee, is limited to statistical data on the number of cases and the types of problems handled. 7.0 CONTRACTOR FURNISHED MATERIALS. The Contractor will be required to furnish all facilities, supplies, and materials to be used during the performance of this contract except those specified in paragraph 8.0 below. 8.0 GOVERNMENT FURNISHED MATERIALS. None. FAR Provision 52.212-2, Evaluation-Commercial Items, applies with paragraph (a) completed as follows: The government will award a contract resulting from this solicitation to the responsible offeror/ quoter submitting the lowest overall total price for the combined base year and all option years of service. The Government will determine the total price by adding the total price for all options (CLINs 0002-0005) to the total price of the basic requirement (CLIN 0001). Pricing should be based on a per month unit price. Invoicing shall be monthly in arrears. Offers/quotes received where the prices are determined to be unbalanced or unrealistic will be rejected.
