- NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with addition information included in this notice. This announcement constitutes the only solicitation: quotations are be ing requested and a written solicitation will not be issued. The reference solicitation number is DAHA23-02-T-0071 and incorporated provisions and clauses are in effect through FAC 2001-06. This is a small business set-aside under NAICS 333924. The Miss ouri Army National Guard has a requirement for a quantity of eleven (11) 6000 pound diesel forklifts,wide profile, air filled tires, Clark Material Handling, model CDP-30 or equal. Each forklift is to be delivered to the individual Operational Maintenance Shops throughout the State of Missouri. Commercial Items, 52.204-6 Data Universal Numbering System (DUNS), 52.212-4; Contract Terms and Conditions?Commercial Items, 52.212-5 Contract Terms and Conditions Required to Implement Statures or Executive Orders?Commercial Items (a)(b)(11)(12)(13)(14)(15)(16)(25)(d)(e), 52.219-6; Notice of Total Small Business Set-Aside. The following DFAR Clauses are applicable: 252.212-7001 Contract Terms and Conditions Requ ired to Implement Statutes or Executive orders Applicable to Defense Acquisitions of Commercial Items; (a) 252.225-7001 and 252.225-7001. The evaluation factors in order of importance are as follows: Technical capability of item offered to meet or exceed the Government?s requirements, vendor?s past performance to meet delivery and price. Each offer should include the following: Item indicating manufacturer, part number, price, and delivery. Submit at least three references to include; name, address and telephone number. Awardee must accept EFT payment and provide account information. A completed copy of the provision at FAR 52.212-3; Offerors Representations and Certifications must accompany your proposal. Due date for submission is 4:00 p.m. CST 15 Ap ril 2002.
