- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. (ii) The solicitation number is F31601-02-T-0010 and the solicitation is issued as a request for quotation (RFQ). (iii) This solicitation document incorporates provisions and clauses in effect through Federal Acquisition Circular 01-06. (iv) This solicitation is 100% set-aside for small businesses. The associated NAICS code is 339112 and the small business size standard is 500 employees. (v) This requirement consists of one line item. (vi) Line Item 0001: LifePak 12 devibrillator/monitor with Adaptiv Biphasic waveform, p/n # LP12BIPHASIC, Mfr: Medtronic Phyisio-Control, 4 each. Also includes: AED option, 4 each; SpO2 Pulse Oximetry with 4ft cable and sensor sample pack, 4 each; Noninvasive blood pressure, with 9ft hose and reusable adult cuff, 4 each; DS100A durasensor adult finger sensor (includes 1 reusable clip); battery support system 2 (includes power cord and instructions), 3 each; LifePak NiCad battery with fuel gauge 2.4amp, 20 each; Basic carrying case for the LP12 (includes shoulder strap, right pouch, and front cover, 4 ach; top pouch, 4 each; left pouch, 4 each; large black pouch, 4 each; removable acrylic screen shield, 4 each. Mfr: Medtronic Physio-control. Or equals are not accepted in this Request for Quotes. FOB shall be destination, and shipping should be included in the unit price. (x) The provision at 52.212-2, Evaluation -- Commercial Items, applies to this solicitation. Paragraph (a) of FAR 52.212-2 is added to read: Price. The contractor certifies that it can meet the delivery date by submitting a quotation. (xi) The provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, applies to this solicitation. The contractor shall return a completed copy of this provision with its quotation. A copy of the provision/clause may be attained from http://www.arnet.gov/far. (xii) The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. Paragraph (o) is changed to read: The contractor shall provide all standard commercial warranties to the government (xiii) The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. The following FAR clauses identified at paragraph b of FAR 52.212-5 is considered checked and is applicable to this acquisition: (11-16), (19i), (21), (24). (xiv) Award can only be made to contractors registered in the Central Contractor Registration web page, http://www.ccr2000.com. (xv) The Defense Priorities and Allocations System (DPAS) is not applicable to this acquisition. (xvi) Commerce Business Daily numbered note 1 applies to this acquisition.
