- Description
- 1.) This is a combined synopsis/Solicitation for Commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, proposals are being requested and a written solicitation will not be issued. 2.) Solicitation #678-87-02. This is and RFP. 3.) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular #6. 4.) The NAICS code is 421450 which has a small business size standard of 100 employees. 5.) The line items and quantities to be purchased are: Line Item 1:Laser System, 1 EA, meeting the following specifications; Laser type: Pulsed; Laser wavelength: 2100nm; Watts: 30 Watt; Pulse duration: 350, 700 microseconds; Pulse Energy: 0.4 to 3.0J; Repetition Rate: 5 to 20Hz 5 to 12 Hz; Visible Aiming Beam: 532 nm (green), 3 maximum, user adjustable (quanta); Electrical Requirements: 15/230V and the ability to change between the 2 voltages; Safety Compliance UL/CSA 2601, EN 60601-1, EN 60601-1-4, EN 60825-1. Including a Standard Warranty:1 year parts and labor and Training Safety, with hands on training to staff 1day on site. Line Item 2: Laser Fiber, 1 EA, meeting the following specifications: Physical: 3 meter fiber, 1000 micron in diameter. Line Item 3: 1000 Micron Holmium Laser Fiber, 1 EA, meeting the following specifications: 3 meter fiber, 1000 micron in diameter. Line Item 4: 200 Micron Holmium Laser Fiber, 2 EA, meeting the following specifications: 3 meter fiber, 200 micron in diameter. Line Item 5: 400 Micron Holmium Laser Fiber, 2 EA, meeting the following specifications: 3 meter fiber, 400 micron in diameter. Line Item 6: 600 Micron Holmium Laser Fiber, 2EA, meeting the following specifications: 3 meter fiber,600 micron in diameter. Line Item 7: Sterilization Tray, 1EA, meeting the following specifications: Autoclavable Fiber Optic, Fibers re-usable. Line Item 8: Quanta Fiber Inspection Tool, 1EA, meeting the following specifications: used to check condition of the fiber before reuse. Line Item 9: Fiber cleaver, 1EA, meeting the following specifications: Used to cut the end of the fiber. Line Item 10: Quanta Fiber Inspection laser, 1EA, meeting the following specifications: Used to verify the condition of the cut. Line Item 11: Holmium Laser Eyewear, 4 EA, meeting the following specifications: Laser glasses, protect staff. Line Item 12: Holmium Laser Goggles, 3EA, meeting the following specifications: Laser goggles, protect staff. Line Item 13: Fiber stripper, 1EA, meeting the following specifications: Fiber stripper 200 micron used to re-condition the fibers. Line Item 14: Fiber stripper, 1EA, meeting the following specifications: Fiber stripper 400 micron used to re-condition the fibers. Line Item 15: Fiber stripper, 1EA, meeting the following specifications: Fiber stripper 600 micron used to re-condition the fibers. Line Item 16: Fiber stripper, 1EA, meeting the following specifications: Fiber stripper 1000 micron used to re-condition the fibers. 6.) A description of the supplies or services required to meet the agency?s needs (including estimated value).The laser will be used for Endourologic applications: lithotripsy, stricture incision, upper tract tumor ablation. The strong absorption and low scattering of Holmium Yag energy ensures destruction of calculi and vaporization of tissues of stricture incision and tumor ablation. The laser must be 30 watts for delivery of higher energy. The laser must also have the capability to change between 300 micro second pulse to 700 micro second pulse. 7.) Items must be delivered FOB destination to the issuing office NLT 30 days ARO. 8.) The provisions of 52.212-1, ? Instructions to Offerors-Commercial? apply to this acquisition. 9.) FAR 52.212-2, ?Evaluation-Commercial Items? is included in this acquisition. The following factors shall be used to evaluate offers: 1) Technical capability of performing as outlined herein; 2) a minimum of 30 watts with the capability of switching between the two power levels. 3) past performance and 4) price. Factors 1 and 2 (technical factors) and past performance, when combined, are significantly more important than price. 10.) Offerors must include a completed copy of the provision at FAR 52.212-3, ?Offeror Representation and Certification-Commercial Items? with their offer. 11.) FAR 52.212-4, ?
