- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information contained in this notice. The procurement is being conducted under Simplified Acquisition Procedures in accordance with FAR Part 13. This announcement constitutes the formal Request for Quotation (RFQ) and a written solicitation WILL NOT be issued. The solicitation document and incorporated provisions and clauses are those in effect through March, 2002 Federal Acquisition Regulation revision, which includes the consolidation of all Federal Acquisition Circulars 05. This acquisition is 100% small business set?aside and the NAICS code is 422120 with a small business size standard of 100 employee. It is the intent of the National Library of Medicine (NLM) to procure the following NLM promotional materials as stated below. The materials, description, and quantity are needed as follows: (1) 135 each - Four Season's brand, model summit, khaki cotton twill low profile unstructured cap with blue bill and beanie. Three-color embroidered with 7,000 stitches in NLM logo on front and web address on back; (2) 50 each - 3/4" custom 24 Kt. Gold plated lapel pin. Die struck with blue cloisonne fill. Standard clutch back; (3) 25 each - 1 inch plastic box with clear top and white bottom, foam insert holds lapel pin; (4) 11,300 each - Bic blue tri-stic pen with maroon trim. Writing on three sides and on the pen clip, black ink; (5) 21,500 each - Bic white tri-stic pen with purple trim. Writing on three sides and on the pen clip, black ink; (6) 500 each - Cobalt blue porcelain china 12-ounce mug with white interior with the NLM logo, printed with 22kt. Bright and frosted gold. Each must be packed in individual corrugated shipping box; (7) 3,900 each - 3" x 4" 3M post-it notes, 50 per pad, white with NLM two-color logo, and web address; (8) 8,150 each - 4" x 2-1/4" purple computer screen sweep, model 1500, with one color imprint. The white printed message reads "MEDLINEplus.gov - A Clear Vision on Consumer Health"; (9) 57 each - Diport USA heavyweight sweatshirt, in gray with navy trim, imprinted in three colors on front, and two colors on back. Poly bagged. Sizes range from LARGE to XXX-LARGE; (10) 333 each - 100% cotton roll-up t-shirt, gray with navy trim. Three color imprint on front and two colors on back. Poly bagged. Sizes MEDIUM to XXX-LARGE; (11) 2,150 each - 14 oz clear cobalt blue, scratch resistant, high impact plastic car travel mug with black splash guard lid measuring 6 1/2" high. Size suitable for most car mug holders. The two-color full-wrap design replicates the NLM homepage header and includes the NLM web address; (12) 322 each - Air-Tex Bright Yellow Drawstring Tote Bag measuring 11w x 11d x 19h. The three-color full-wrap logo design includes the NLM web address; (13) 3,547 each - Sweda brand Silver color double flipper calculator with one color imprint on outside and a one color imprint on hinge above readout window; (14) 500 each - XJD Mouse Pad - 8" x 7 1/2" white, blue, and black rubber-backed, glossy-finished mouse pad with NLM logo and web address; (15) 100 each - Binaco LS-710 Red insulated PVC lunch bag with Velcro closure, front slip pocket and webbed handle. Screened in three color on front pocket with NLM logo and website. Vendor must maintain an inventory of products and be able to provide order fulfillment from a local warehouse for individual orders placed via the World Wide Web. These orders must be filled within 3-5 business days. Individual orders will range from one to multiple items. This inventory will be the sole responsibility of the vendor. Vendor will be responsible for maintaining order records and invoice once a month for payment for product, handling and shipping. Additional cost for storage and shipping must be priced separately. All proposals received within 10 days from the date of publication of this synopsis will be considered by NLM. Responses must be in writing and must be received in this office by April 26, 2002 from the date of publication of this notice, and must include pricing information and meet the criteria requirement. Responses must provide ALL or NONE of the items above. All responses from responsible sources will be considered.
