Th is is not a request for a quote or proposal, or an invitation for bid. The intent of this pre-solicitation notice is to identify potential offerors capable of performing a performance-based hospital housekeeping service contract who meet the minimum Corpo rate Experience required by US Army Medical Command, for Hospital Housekeeping Services. This requirement is for hospital housekeeping and linen distribution services at Brook Army Medical Center and 19 outlying buildings located at Fort Sam Houston, TX. The NAICS Code for this proposed procurement is 561720 and the annual size standard is $14.0 Million. Method of acquisition will depend on Industry Response. Contractor Corporate Experience requires that the firm (company) have gained experience in hosp ital housekeeping as a result of being regularly engaged in the business of providing housekeeping services in healthcare/patient care environments (e.g., inpatient, outpatient, ancillary, surgical, and dental care). The minimum level of acceptable experi ence required is 24 months within the previous 36 months from the date initially established for submission of proposals. Periods of creditable performance may have been incurred at more than one site for at least 12 consecutive calendar months at each si te and could not have been gained at two or more sites concurrently. The Statement of Capability (SOC) will be the basis for the Government?s decision to determine any potential offerors who meet the requirement for Corporate Experience. The SOC must be on 8 x 11 standard bond paper, tabbed in the order listed below: (1) Full name and address of the firm; (2) Year firm established; (3) Names of two principals to contact (including title and telephone number); (4) Positive statement of eligibility under NA ICS Code 561720, Janitorial Services with average annual gross revenue of not more than $14.0 Million for the last three completed fiscal years; (5) A statement of contractor experience as a company in providing hospital housekeeping services, especially t hose services to institutions of similar size and scope of the hospitals covered by the solicitation; (6) Provide a list of all contracts and subcontracts similar in nature (size and complexity) to the acquisition, awarded or performed during the past thre e (3) years. Contracts listed should include those entered into with the Federal Government, state or local governments and commercial customers. Offerors shall include the size of facility (bed capacity and square footage) where services were performed. For each contract listed, provide the names and telephone numbers of customer?s technical representative, customer?s contract administration activity, and the administrative contracting officer. The information shall include contract numbers, contract a ward dates, contract types and total prices. The description must include the following for the previous 3 years: (1) A narrative description of each contract listed and why you consider it similar to the effort required by the hospitals under this solic itation. (2) Information on all contracts that have been terminated, in whole or in part, for default during the past 3 years, including those currently in the process of termination. The offeror may provide information on problems encountered on the cont racts/subcontracts identified and provide description of any corrective actions taken by the offeror to resolve those problems.
