- Treasure Island Job Corps is requesting a proposal to provide Trash, Refuse, and Recycling Collection Services for our Facilities. These services will include the buildings known to the Treasure Island Job Corps as follows: Building #containers Capacity Type of Waste Frequency of pickup 363 2 2-yard Regular Waste 4 times per week 1 191"*85"*94" Cardboard As needed 364 2 2-yard Regular waste 4 times per week 1 191"*85*94" Cardboard As needed 365 2 2-yard Regular Waste 4 times per week 366 2 2-yard Regular Waste 4 times per week 1 220 lbs Paper Weekly 367 2 2-yard Regular Waste 4 times per week 368 11 2-yard Regular Waste 4 times per week 3 220 lbs Glass Weekly 1 220 lbs Tin, Aluminum, Recycle Weekly 369 5 2-yard Regular Waste 4 times per week 1 220 lbs Paper Weekly 1 220 lbs Tin, Aluminum, Recycle Weekly 1 220 lbs Glass Weekly 442 2 2-yard Regular Waste 4 times per week 3 220 lbs Paper (white) Weekly 1 220 lbs Newsprint paper Weekly 2 220 lbs Glass Weekly 487-489 2 2-yard Regular Waste 4 times per week All interested vendors will follow the following procedure: Step 1: Contact Ezequiel Vega, 415-277-2429, to notify him of interest on bidding and obtain a copy of the RFP packet. The packet will include an application to the City and County of San Francisco for a permit to operate in a Federal Facility. Step 2: Fill out the application upon receipt and submit it to the City and County of San Francisco Department of Public Environmental Health Management. The application will be sent to prospective bidders upon request along with a copy of RFP and response requirements and guidelines. Send all applications to Henry Louie, REHS, MPH by mail at: ATTN: Henry Louie, REHS, MPH LEA Program Manager 1390 Market Street, Suite 212 San Francisco, CA 94102 Step 3: Upon receipt of a letter from the City and County of San Francisco pre-qualifying your company, submit your proposal to Treasure Island Job Corps by COB 06/07/2002. Proposals postmarked later than this date will not be accepted. Please send all proposals to Ezequiel Vega by mail at: ATTN. Ezequiel Vega 655 H Avenue Building 442 San Francisco, CA 94130 The response must include documentation from the City and County of San Francisco (i.e. a letter) indicating the prospective vendor's pre-qualification to obtain a permit as a Federal Facility Trash and recycling collection provider according to the City and County of San Francisco guidelines. Therefore, all prospective vendors must immediately send their application to obtain this letter to the address mentioned in step 2. The proposal will have to structured including the following information: 1. Business Name and Address 2. Name and Title of representative submitting this proposal 3. Date submitted 4. Description and itemization of services to be performed under this proposal to include: Building # # Containers per building Capacity of Containers Frequency of Collection Type of Waste collected per container 5. Government qualifications: All items in the above list will be awarded to one offeror. 6. Offeror qualifications: Must posses permit to operate in a federal facility after contract award for the services requested. Must demonstrate the ability to provide the services requested. Step 4: All bidders will be notified of the award of the contract by COB on 06/28/2002. The successful bidder must be ready to start services on 08/01/2002.
