- ALL REQUESTS FOR SOLICITATION PACKAGES MUST BE IN WRITING, E-MAILED OR BY FAX (765-677-3106). The VA Northern Indiana Health Care System, Marion, Indiana has a requirement for a Community Based Outpatient Clinic (CBOC) services to cover primary and preventive medical care and continuity of care to eligible beneficiaries of the VA Northern Indiana Health Care System (VANIHCS) in the Muncie (Delaware County), Indiana area. The eligibility of veterans will be determined by the VANIHCS and confirmed in writing to the provider. The contractor shall provide health care clinical providers, medical facilities, medical administrative functions to fulfill the level of care and rage of services. Compensation will be made through monthly capitation payments in accordeance with the terms and conditions of the proposed contract. The period of performance will be for a base year with four (4) one-year option periods, which may be exercised at the option of the Government. North American Industry Classification System (NAICS) code is 621399, $6.0 million average annual receipts for the last three years. The solicitaiton will be issued May 20, 2002.
