- Description
- The Great Lakes Acquisition Center (GLAC) is soliciting quotations to provide PATIENT SATISFACTION SURVEYS for the VISN 12 which consist of 7 medical centers. Anticipated period of performance is a base year, May 1, 2002 through April 30, 2003 and 2 option years. Contract type is firm-fixed price. Anticipated award effective date is May 1, 2002. Electronic version and hard copy of solicitation will be available on or about April 17, 2002. This solicitation is issued under the authority of Public Law 104-262 and 38 USC 8153. This procurement is being conducted under the Subpart 13.5 test program where simplified acquisition procedures apply and the conduct of the procurement will be in accordance with Parts 12, 13, or 15 of the FAR or some combination thereof. Contract award is subject to availability of funds. NAICS number is 541611. Electronic solicitation may be obtained FREE at the following URL: http://www.bos.oamm.va.gov/ Requests for hard copy solicitation must be in writing to the GLAC Contact Point and accompanied by a check for $5.00 made out to the Department of Veterans Affairs. No telephone requests will be accepted. Any amendments issued to this solicitation will also be available at the URL address. Offerors are advised that they are responsible for obtaining amendments.
