Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation 502-32-02 is a Request for Proposal for one (1) phlebotomist four (4) hours each weekday, 7:30 AM to 11:30 AM, for a community based out patient clinic. The procurement is unrestricted and all responsible sources may submit an offer for consideration. Offers shall be considered only if clear and convincing evidence is provided documenting that the offeror can meet the requirements of the Government--qualifications, capabilities and timeliness. Services are needed beginning June 1, 2002. The solicitation documents and incorporates provisions and clauses in effect through FAC 2001-06 and VAAR # 21. Applicable NAICS code could be 561330 with $5.0 million annual sales limitation for small business, 621511 with $10 million annual sales limitation for small business, or 622110 with $25 million annual sales limitation for small business. The ability to provide one person to draw and process blood and collect/prepare other specimens in accordance with all contract requirements - with one million dollars professional liability is required. Any qualified source ? hospital, laboratory, employment placement agency or other offering Phlebotomists, Medical Technician, Emergency Medical Technician or others qualified may submit an offer for consideration. Anticipated award is one firm fixed price, indefinite quantities contract. SERVICES AND PRICE/COST: The Contractor shall furnish one (1) experienced and qualified Medical Technician/Phlebotomist to collect, process and prepare for shipping, laboratory specimens for outpatients at the Department of Veterans Affairs Medical Center (VA or VAMC)Jennings Community Based Outpatient Clinic (CBOC or Clinic), 1907 Johnson Street, Jennings LA 70546, four hours per day, Monday through Friday, excluding Government holidays. Average daily patient load is thirty (30). Services require adequate computer skills to input patient and test request and related data into VA's computerized medical records system. Cost/Reimbursement may be hourly or per unique patient. Contract term includes a base year and four (4) optional one-year provisions. Services shall be provided in accordance with all terms, conditions and requirements of this solicitation,Occupational Safety and Health Administration (OSHA), VA requirements and Joint Commission on the Accreditation of Healthcare Organizations (hereinafter referred to as JCAHO). One tech is required daily with backup services available. Only VA oriented, pre-approved substitutions may be made in the absence of the primary technician providing contract services. Offerors must provide pricing for a base and four optional one year terms, per hour, based on 1000 hours each contract term OR provide pricing for a base and four optional one year terms based on 7800 uniques base year, with a 200 patient increase each of the option years. Provide a total for the five years. Patient quantities are estimates only and not guaranteed. Specimens include venous whole blood, urine, and stool. Patient population at the Clinic includes male and female veterans, ages 18-65, and senior adults over the age of 65. Patient load varies up to 40 per day. Estimated daily average is 30 patients. Contract technician must have specialized experience and training in phlebotomy. Collection and preparation of samples shall be in accordance with established industry and laboratory protocol. Licensed Practical Nurses skilled in drawing blood and related duties, Phlebotomist, or a Medical technician may be considered. Computer skills required to retrieve orders, enter patient and test data into VISTA, utilize barcode labels, access system for results, status, and related issues. Specific training will be provided to individual with laboratory & general computer knowledge. The tech shall operate, maintain & clean centrifuge equipment, perform centrifugation of specimen and aliquoting for storage and shipment, determination of acceptable specimens, analyze, evaluate and correlate proper sampling techniques, met current requirements, use techniques to prevent transmission of disease, maintain and monitor general lab supplies/inventory. VA shall provide all equipment and supplies. Must have communication skills and expertise to conduct personal contacts including face-to-face, telephone and written. Duties may include answering telephones for lab related questions, and referral of unanswered questions to the appropriate sources. He/she shall independently receive patients and care for his/her needs during blood draw, maintain cleanliness and order of the designated blood collection area in accordance with VA infection control guidelines. The contract Medical Technician shall possess the knowledge and skills necessary to provide patient care that is appropriate to the cognitive, physical, emotional and chronological maturation needs of the adult through senior adult patient. He/She shall have a high degree of skill in drawing blood. The technician must possess at least six months of general experience and six months of specialized experience drawing blood and preparing laboratory specimens. General experience is any type of work that demonstrates the ability to perform the work of the position or experience that provides a familiarity with the subject matter or processes of a broad subject area of the occupation. Specialized experience is technical medical laboratory support work such as collection of clinical laboratory specimens and/or performing lab tests & examinations and preparing reports of findings that required application of the methods and techniques. Education at a school for Medical Laboratory Technicians or at a school for Phlebotomy may be substituted for experience. Technician with current Louisiana license as a Laboratory Assistant and/or certification as a phlebotomy technician (PBT), (An American Society of Clinical Pathologists (ASCP) is preferred. Knowledge required: Knowledge of clinical laboratory concepts, terminology and methodology to select the proper sampling techniques and specimen collection devices for the tests requested; Knowledge of clinical laboratory and hospital accreditation requirements regarding correct specimen and patient identification, Proper preserving and handling of specimens, Proper disposal of incorrect/rejected specimens and other materials used in the work and considered contaminated, Knowledge of the principles of asepsis and other methods to instruct patients and clinic staff on the techniques for collecting laboratory specimens, Knowledge of laboratory principles and methodologies such as fasting, Knowledge of specimen storage requirements to properly process and prepare specimens for transportation , Knowledge of computer entry techniques for specimen accessioning. (Specific training will be provided), Knowledge of clinic procedures, laboratory procedures, read, write and speak basic English with knowledge of transmissible diseases, body and venous anatomy, lab supplies, & patient reactions. Cardiopulmonary Resuscitation Certification & documentation of current TB skin testing and record of Hepatitis B vaccination with current titer or signed declination is essential. Patience, tact, professionalism, and the ability to enlist the cooperation of difficult patients, treat employees, patients and visitors with respect and demonstrate appropriate and proactive interpersonal and other job related skills with appropriate skills, knowledge and attitude to work independently. Guidelines include agency regulations, accrediting regulations (JCAHO), technical manuals, journals, OSHA, instruction and procedural manuals or media visual aids for use in conjunction with his/her knowledge and skills. In the performance of official duties, the Contractor technician has regular access to printed and electronic files containing sensitive patient information that is protected under the provision of the Privacy Act of 1974, and other applicable laws, federal regulations, Department of Veterans Affairs statutes and policies, VHS&RA policy. The employee is responsible for protecting information from unauthorized release, loss, alteration, or unauthorized deletion, and following applicable regulations and instructions regarding access to computerized files, release of access codes, etc., as set out in the hospital's Automated Data Processing Agreement. In accordance with Veterans Affairs Information Letter (IL) 90-01-6 and Handbook 0710 (available on the internet at <http://vaww.va.gov/pubs/handbooks/general-Management/0710h.pdf> and <http://vaww.va.gov/miss/laws-regs/national/0710.pdf>, a background security check must be provided on contract employees with access to VA's computer system at the Contractor's expense. Job requires regular standing, walking, bending, occasional lifting of patients and supplies, reaching, kneeling, and pushing. Exposure to contagious diseases, caustic and vaporous reagents require safety precautions and protective clothing. Federal Holidays (10) are - New Years Day, Martin Luther King's Birthday, Washington's Birthday, Memorial Day, Independence Day, Labor, Day, Columbus Day, Thanksgiving Day, Veterans Day, Christmas Day, and any other day specifically declared by the President of the United States to be a National Holiday. No services are required on holidays. Contractor shall provide a replacement laboratory technician if the regularly scheduled phlebotomist is unable to report for duty. Any delay of more than 15 minutes must be reported to the designated VA staff at Jennings CBOC and a replacement must be provided no later than 8:30 AM. Contract period is June 1,2002 through May 31, 2003. There are four optional one-year provisions which require formal notification of acceptance. This contract is subject to the availability of VA funds. Costs are all-inclusive, with no additional pay for travel, lodging, credentialing/licensing costs, holidays, vacation, sick time, and Malpractice Insurance. No other consideration shall be given for any reason. Billing address is Integrated Financial Operations (04), Department of Veterans Affairs, PO Box 69004, Alexandria LA 71306-9004. Invoices shall include the contract number, number of hours worked or patients (names) seen during each billing cycle by the Phlebotomist, and the date services were provided. Billing may be biweekly or monthly in arrears. ACH Vendor/ Miscellaneous Payment Enrollment Form ? SF 3881 must be filled out and returned to the address on the form prior to first billing to accommodate electronic funds payments. Contracting Officer's Technical Representative(COTR) will be designated prior to commencement of work and act in behalf of the Contracting Officer in a limited capacity. The Contracting Officer must make any changes in the COTR delegation or duties. An information copy of the designation will be provided to the Contractor. Contract Administration: The Contracting Officer is the only person authorized to approve changes or modify any of the requirements under this contract. Only the Contracting Officer is authorized to make commitments or issue changes that will affect price, quantity or quality of performance of this contract. Clinical or other medical records of VA beneficiaries shall remain the property of VA with no information used in any capacity by the Contractor or its representatives. The contractor shall be responsible for protecting the personnel furnishing services under this contract. The contractor shall provide Worker?s Compensation, Professional Liability Insurance, Health examinations, Income Tax withholding, and Social Security payments. Personnel shall not be considered VA employees for any purpose and shall be considered employees of the contractor. The resources specified may be changed by written modification to this contract prepared and approved by the VA Contracting Officer. Resources specified shall be furnished to the VA by the contractor when requested by means of a written request authorized by the Contracting Officer. When contract services are provided by a facility not required to maintain JCAHO accreditation or other regulatory requirements regarding worker competency, the contractor shall perform the required work in the same standards as JCAHO standards. The contractor is required to develop and maintain for each Contractor employee working under subject contract copies of Credentials and qualifications for the job including, but not limited to, continuing education for the last two years, Current licenses/certifications, Social Security number, date of birth, Immunization Record, current CPR certification, competency assessment checklist (an assessment of knowledge, skills, abilities, three (3) work related references to include last previous employer and behaviors required to perform a job correctly and skillfully; includes knowledge and skills required to provide care for certain patient populations, as appropriate) and a current performance evaluation. A background criminal check on each presented to work under subject contract must be completed at contractor expense according to IL 90-01-6(VA may also do a background and criminal check) with current copies at the time of contract award for each person being presented and annually on the anniversary date of contract award, or upon request. Monitoring: The COTR or other VA employee(s) shall develop a monitoring system to ensure Contractor attendance, quality of services provided, billing accuracy, performance for contract compliance, knowledge, skills and behavior and immediately notify the Contracting Officer of any noncompliance or unacceptable behavior. A sign-in/sign-out log shall be maintained by VA staff to document attendance. Insurance: In accordance with Veterans Acquisition Regulation clause 852.237-7, the Contractor shall maintain and provide confirmation of Professional Liability Insurance in the amount of at least one million dollar per occurrence for the duration of the contract. Wage Determination # 1994-2505, revision 18, 3/15/02, applies. Award: Offerors shall be evaluated on technical capabilities, past performance and cost. Technical and past performance when combined are equal to cost to provide the Best Value to the Government. Technical includes all qualifying factors. Past performance includes all prior work places, similar requirements, and references. Costs may be provided per hour or per patient. Award may be made based on documentation provided with offer. Submit all backup with offer.
