- NOTICE:::Solicitation Number SP0450-02-R-4967 is hereby revised and modified to read Solicitation Number: SP0450-02-R-5147 ARG02035054601, NSNS 6840-00-238-8115, 6840-00-255-0471, 6840-01-358-4336, Disinfectant, Calcium Hypochlorite, D.O.T. Class: 5.1, Shipping Name: Calcium Hypochlorite, Hydrated Mixtures UN2880. EPA Registration/Labeling Required. Unit Polyethylene Bottle contains 5 lbs. P/N EPA Reg. No. 748-239-5550, P/N Cal Hypo Granules, EPA Reg. No. 69681-6, P/N Calcium Hypochlorite or Induclor+, EPA Reg No. 748-296 or 748-239 P/N HTH Granular 75 or HTH Dry Chlorinator Granular, EPA Reg No. 1258-1170 or 1258-1069 or P/N: Calcium Hypochlorite, Granular EPA Reg No. 1258-1069-72932. Final EPA Labels that will be applied to the Packaging and OSHA MSDS shall be submitted with Quote. EPA Labels shall have EPA approved directions for Disinfection of DrinkingWater (Emergency/Public/Individual Systems), under Directions for use. Other EPA approved uses (I.E. Swimming Pools, Etc.) may also be included. AWWA B 300-99 applies. This Solicitation will result in an Indefinite Quantity Contract for One (1) year with an option to extend the terms of the contract for Four (4) years. Sole Source Item . ARO for Stock Items is 30 days, ARO for Direct Vendor Deliveries is 15 days. Solicitation is Unrestricted. Guaranteed annual minimum dollar is $15,044.48. Maximum dollar amount per year is $324,636.88. Automated Best Value System (ABVS) applies. The final contract award decision may be based upon a combination of price, past performance and other evaluation factors as described in the Solicitation. The Solicitation will issue on May 6, 2002 with a closing date of May 30, 2002.
