Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This procurement is conducted under simplified acquisition procedures in accordance with FAR Part 13. This announcement constitutes a formal Request for Quote (RFQ) and a written solicitation WILL NOT be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition 01-06. RFQ 546-39-02 should be referenced on all offers. The North American Industry Classification for this procurement is 492210; small business size standard is $21.5 million. The Government intends to award a contract for a base year with four (4) one year options. This procurement is unrestricted. Requirement: 1. The VA Medical Center in Miami, Florida has a need for a Driver and a Refrigerated Truck from with a freezer compartment, if possible. The truck can be from 16 to 20 ft. 2. Truck shall be equipped with straps for securing mobile food carts which will contain the prepared meals. 3. Refrigerated contents are to remain between 35-40 degree F at all times. Delivery of prepared meals will go to West Palm Beach VA Medical Center 3 days a week (Tuesday, Thursday & Saturday) Six (6) hours per day from 7am - 1pm, approximately 160 miles per day round trip. 4. The driver shall posses at a minimum, a Florida safe driver class B license. 5. The driver shall present a neat and clean appearance to include the uniform (if applicable) and general clothing in good repair. 6. The contractor shall be responsible for the truck and driver as well as provide liability insurance for the contents being transported. 7. The contractor shall provide coverage for sick leave and other absences. Scope: 1. Inside Pick up and Delivery activities are required. 2. Truck is to be swept and cleaned between each delivery. Spills are to be reported to receiving site and cleaned before next load. The truck shall be sanitize regularly and be free from all transport debris. 3. Driver shall report with the truck, refrigerated and ready to begin transporting prepared food product from Nutrition & Food Service located in the 2nd floor location. With the help of a VA employee, the driver will load the truck and secure carts. 4. Certify load contents. Seal truck, Leave VAMC Miami NLT 8 AM, Arrive VAMC WPB NLT 10 AM. Off-load from WPB dock to basement kitchen storage area. Obtain sign invoice and return with Miami load. 5. Load freezer items and return racks and pans. Certify load contents. Seal truck. Leave VAMC WPB NLT 11 AM. Arrive VAMC Miami NLT 1 PM. Off-load from Miami dock to 2nd floor kitchen location. Submit invoices to receiver. 6. Truck seal to remain intact until broken by receivers at WPB and at MIAMI. 7. Delays in transport should be communicated by company to receiving site. 8. Drivers shall have evidence of HACCP training, i.e. food handling/transport procedures. (The VA will provide orientation if scheduled one week in advance.) 9. Company must provide records of driver?s safety training, certifications and licensure as required by law. 10. Contractor will be trained as required by VA reg. Any contractor who receives a contract from the Federal Government in the amount of $25,000 or more, or any subcontractor who receives a contract from such a covered contractor in the amount of $25,000 or more, must file a VETS-100 Report on an annual basis.
