Description
(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. Proposals are being requested and a written solicitation will not be issued. (ii) This combined synopsis/solicitation is issued as a Request for Proposal (RFP #927593). The Federal Bureau of Investigation (FBI) intends to purchase preventative maintenance for "Liebert Uninterruptible Power Supply. (iii) This solicitation document and incorporated provisions and clauses are those in effect through the FEDERAL ACQUISITION CIRCULAR 2001-06. (iv) A firm-fixed-price purchase order will be issued as a result of this combined synopsis/solicitation. (v) The RFP is for the following CONTRACT LINE ITEM NUMBER (CLIN): CLIN 0001: Quantity: 001, FY02: Preventative maintenance is to be performed on the Uninterruptible Power Supply CLIN 0002: Quantity: 001, FY03: Preventative maintenance is to be performed on the Uninterruptible Power Supply CLIN 0003: Quantity: 001, FY04 Preventative maintenance is to be performed on the Uninterruptible Power Supply CLIN 0004: Quantity: 0001, FY05 Preventative maintenance is to be performed on the Uninterruptible Power Supply CLIN 0005: Quantity: 0001, FY06 Preventative maintenance is to be performed on the Uninterruptible Power Supply This solicitation is to be awarded to include all the items listed above. Note CLINs 0002 thru 0005 are for 4 option years. (vi) Due to the size of the statement of work, it cannot be published here. Request a copy of the Statement of Work from: Federal Bureau of Investigation, Attention: Michael Konchesky, 1000 Custer Hollow Road, Clarksburg, WV 26306, (304) 625-4031. If Michael Konchesky is not available, then contact Glenn Armfield at (304) 625-2492. (vii) Maintenance shall be performed at: Federal Bureau of Investigation, Criminal Justice Information Services Division (CJISD), 1000 Custer Hollow Road, Clarksburg, West Virginia 26306. (viii) FAR 52.212-1, Instructions to Offerors -- Commercial Items is incorporated by reference and applies to this acquisition. ADDENDUM FAR 52.212-1 Para. (B) Submission of offers. Submit signed and dated offers by 12:00 Noon Eastern Standard Time on May 31, 2002, to the Federal Bureau of Investigation, Information Technology Contracts Unit, Attn: Michael Konchesky, Module B-3, 1000 Custer Hollow Road, Clarksburg, WV 26306 or facsimile at 304-625-5391. (ix) FAR 52.212-2, Evaluation -- Commercial Items is incorporated by reference and applies to this acquisition. ADDENDUM FAR 52.212-2 Para. (A) The following factors, in descending order of importance, shall be used to evaluate offers: past performance, technical approach, and price. For past performance evaluation, offerors shall provide names, addresses, and telephone numbers of three customers, Government or commercial, that have utilized the offeror's services for the same or similar item during the past one year. Contract will be awarded on a Lowest-Price Technically Acceptable. The proposal submitted must be separated into two parts. Part one will contain the past performance information and technical information. Part two will contain the pricing information only. (x) Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications--Commercial Items with offer. Offerors shall ensure that all Representations and Certifications are executed and returned as required in this synopsis/solicitation. The resulting contract will be firm, fixed price (FFP). The following provisions also apply to this acquisition: FAR 52.233-3, Protest after Award FAR 52.222-21, Prohibition of Segregated Facilities; FAR 52.222-26 Equal Opportunity; FAR 52.222-35, Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era; 52.222-36, Affirmative Action for Workers with Disabilities; 52.222-37, Employment Reports on Disabled Veterans and Veterans of the Vietnam Era; 52.225-3 Buy American Act ; 52.232-34, Payment by Electronic Funds Transfer -- Other than Central Contractor Registration; 52.222-41 Service Contract Act of 1965; 52.222-43 Fair Labor Standards Act and Service Contract Act; 52.222-47 SCA Minimum Wages and Fringe Benefits Applicable to Successor Contract Pursuant to Predecessor Contractor Collective Bargaining Agreements.
