Description

This is a combined synopsis/solicitation for commercial items or services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. This solicitation is issued as a Request for Quote. The solicitation clauses and provisions are those in effect through Federal Acquisition Circular 97-27. The Standard Industrial Classification (SIC) Code for this acquisition is J099 and the NAICS Code is 811212. The Food and Drug Administration (FDA), Center for Veterinary Medicine (CVM) has a requirement to acquire a standard maintenance coverage service agreement for the following pieces of equipment; SAMS Computer Systems/model no. CM4000/Qty 3; Autoreader/model no. V3029/Qty 4; Sensitouch/model no. V3027/qty 3; Aris/model no. V3090/Qty 2; Autoinoculator/model no. V3010/Qty 2. Maintenance period is for one year and should include an unlimited number of service calls Monday-Friday, 8:00 am - 5:00 pm local time. Excludes weekends and holidays. All labor, travel and parts included. Response time within four (4) hours. Technical Support Hotline available 24 hours/day, 7 days/week. Includes one (1) preventative maintenance inspection. The following FAR clauses shall appply to this solicitation: 52-212-1 Instructions to Offerors Commercial, 52.212-4 Contract Terms and Conditions Commercial Items, and 52.232-22 Electronic Funds Transfer. It is anticipated that this requirement be awarded on a sole source basis to Trek Diagnostics Systems, Westlake, OH. It is the belief of the CVM that this company is the only source with the expertise, personnel, and supplies/parts on hand to fully maintain the above-referenced equipment. All responsible sources that can provide and meet the above-referenced requirement shall submit written quotations by mail or fax 301/827-7039.
