Description
This is a combined Synopsis/Solicitation as authorized by FAR 13/5 Test Program. AFMOA/SGZZ is a forward operating agency of the United States Air Force Surgeon General (USAF SG). SGZZ's mission is to enhance the readiness of a fit and healthy force through effective disease and injury prevention and health promotion. SGZZ needs to acquire contractor services to develop, test and help deploy a software application for the assessment, diagnosis and management of individuals in the Alcohol and Drug Abuse Prevention and Treatment Program (ADAPT). This program involves both patients who are referred to the ADAPT Program following a substance-related incident as well as self-referrals. This application must be a client-based application that is loaded for use on local computers or LANs and has the capability for transmission of data to a central data collection and storage database site that will be used by all AF bases. The application must produce both patient and provider feedback output reports as well as incorporate a patient tracking function to manage ongoing treatment cases (see Attachment #1 SUAT Clinical and Functional Requirements Document and Attachment #2 SUAT Output Report File). Roughly 5000 patients (cases) per year are assessed across the USAF, with varying numbers of patients at any given base. NOTICE: Funding source has not yet been identified. As a result it is possible that no contract award will be made. PROPOSAL SUBMISSION The Government will request proposals for this task utilizing the ITSS Web-based Order Processing System. To receive a potential Request For Proposal (RFP), the potential contractors shall register their organization and their authority to submit quotes at the following Web Site: http://it-solutions.gsa.gov The contractor shall submit a written cost and technical proposal for consideration before the due date and time for this RFP. The contractor shall submit a payment schedule as part of the cost proposal. In addition, the contractor shall submit these proposals electronically utilizing the ITSS system. At the completion of negotiations, the contractor shall ensure his cost and technical proposal reflects agreements and, if appropriate, submits revised proposals to the ITR via ITSS.
