- NA This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are be ing requested and a written solicitation will not be issued. The solicitation number as shown in this announcement is a Request for Quotations (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acq uisition Circular (FAC) 01-06. This solicitation is unrestricted and the NAICS is #621340. The service is for medical monitoring for all full time support personnel of the New Mexico Army National Guard. Complete Statement of work will be provided upon request. Contractor must be board certified in Occupational Medicine and must be full service provider of Occupational Medicine. Contractor must be able to service the whole state and have offices in the major cities such as Las Cruces/El Paso, Santa Fe and Albuquerque. The Contractor must ensure all procedures are conducted in accordance with applicable U.S. Department of Labor Occupational Safety and Health Administration (OSHA), National Institute for Occupational Safety & Health (NIOSH), American Coll ege of Occupational and Environmental Medicine (ACOEM), American Board of Preventive Medicine (ABPM) and American Occupational Health Nurse standards and procedures. SPECIFICATION/PRICING LIST The contractor agrees to provide the following in office services at the agreed upon prices. All other tests/services required will be priced at the time of service. (Noise & Hearing Conservation)$___________________ B - Reading$___________________ Pulmonary Function Test (Respiratory Program)$___________________ Respiratory Physical$___________________ Exam to include Respirator Clearance, physician review of completed Appendix C to 1910.134, OSHA Respirator Medical Evaluation Questionnaire Mandatory), Carbon Monoxide Test$___________________ Asbestos Physical$___________________ Job Placement Analysis/ Functional Weighted Activity$___________________ (includes four to five components (20-25 minutes), 3 lifts, carry, push/pull) Tuberculin Skin Test (PPD)$___________________ 5-Panel Urine Drug Screen$___________________ (ONLY given for new-hire employee Preplacement Exams & individuals requiring DOT Commercial Vehicle Driver License) Breath Alcohol Test (EBT) $___________________ Contract is for five years, base year and four options years. Rate adjustments will be negotiated for each option year. Billing will be done monthly after review and acceptance of work. Wage rates # 94-2361 are applicable to this contract and will be in corporated upon contract award. This solicitation will employ a best value source selection process. The best value evaluation factors are, in descending order of importance, 1) Technical capability of the contractor to meet the Government requirement, 2 ) Board certification in Occupational Medicine and proof of being full service provider of Occupational Medicine, 3) Locations of Facilities, 4) Price. Technical and past performance, when combined, are significantly more important than price. This con tract will be a firm fixed price contract. Contractors shall include a completed copy of the provision at FAR 52.212-3 Offeror Representation and Certifications Commercial Item with its offer.
