Description
The National Institutes of Health (NIH), National Institutes of Neurological Disorders and Stroke (NINDS), Medical Neurology Branch (MNB), proposes to enter into a sole source contract with Dr. Steven Bielamowicz, 1500 Twisting Tree Lane, McLean, Virginia 22101 for out patient otolaryngological services. The North American Industry Classification System code is 541690 and the business size standard is $5M. The MNB requires a uniquely qualified professional specialized in providing neurolaryngologic services, skilled in the electrical stimulation of superior laryngeal nerve and multiple muscle hooked wire electromyography for treating patients with laryngeal motor control problems. The individual shall serve as a physician responsible for patient care of participants in a study of the acute effects of dysphonia. In particular, the contractor will be responsible for conducting history, physical examinations, and screening of participants for clinical protocols; perform botulinum toxin muscle injections; laryngeal neurophysiological examinations involving procedures for laryngeal electromyography (EMG), electrical stimulation of the superior laryngeal nerve, air pressure stimulus to the larynx, servomotor stimulation of the larynx and bilateral temporary chemical nerve block of the superior laryngeal nerve; and participate in the development of a neuroprosthesis for neuromuscular stimulation for airway protection during swallowing. Dr. Bielamowicz is uniquely qualified and highly specialized in the area of laryngeal motor control problems. He has over ten (10) years experience in the field of neuroloaryngology. He is highly skilled in the electrical stimulation of the superior laryngeal nerve and multiple muscle hooked wire EMG. He is familiar with the research being conducted since he was previously trained at NIH, and he is knowledgeable of this type of research performed at the NIH Clinical Center. The Period of Performance is June 1, 2002 through September 30, 2002, for one day a week during the period of performance. FAR Clause 52.217-8 Option to Extend Services, November 1999, applies to this order. The Government may require continued performance of this service within the limits and at the rates specified in the contract. Based on the aforementioned requirements, skills, and abilities, Dr. Bielamowicz is the only source known that is uniquely qualified to meet the needs required for this effort. The requirement will be of a fixed price nature, procured in accordance with FAR 6.302-1. Only one responsible source and no other services will satisfy agency requirements. This is not a solicitation for competitive quotations. However, if any interested party believes it can provide the above service, it may submit a statement of capabilities. The statement of capabilities and any other furnished information must be in writing and must contain material in sufficient detail to allow NINDS to determine if the party can perform the requirement. The capability statements must be received in the contracting office by 1:00 PM EDT, (local Washington, D.C. time) May 23, 2002.
