Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format prescribed in FAR subpart 12.6, as supplemented with additional information included in this notice. The VA Healthcare network Upstate New York comprising of the Stratton VA Medical Center, Albany, NY, VA Medical Center, Syracuse, NY and VA Western New York Healthcare System, Buffalo, NY is entering into negotiations with Arrow International for Antiseptic-impregnated triple-lumen catheterization kits with sharps safety features. Features include Central venous Catheterization Kit with antiseptic surfaces (external & internal); Triple (or multi) lumen radiopaque polyurethane indwelling catheter (7 Fr x 20 cm); 25 gauge needle; 22 gauge needle; 18 gage radiopaque catheter over 20 gauge introducer needle; 18 gauge thinwall needle; 3CC syringe; two 5CC syringes; Dual purpose spring-wire guide (straight soft tip on one end- tip on other); #11 blade safety/retractable scalpel; 3-0 suture with needle, 2% chlorhexidine gluconate/alcohol prepping solution; vessel dialator; 2 line clamps, Oversized fenestrated drape, CSR wrap; Locking sharps disposal cup and Staple anchoring system. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. All interested parties are requested to give written notification including all pertinent data consisting of specifications and descriptive literature to include independent studies to support the products effectiveness and toxicology review/reports to demonstrate safety of proposed product. The proposed contract action for which the Government intends to solicit and negotiate with only one source is under the authority FAR 6.302. Interested persons may identify their interest and capability to respond to the requirements or submit proposals. The North American Industry classification System (NAICS) Code applicable to this solicitation is 339112. This solicitation document incorporates Federal Acquisition Regulation (FAR) provisions and clauses in effect through Federal Acquisition Circular 2002-07 and VA Acquisition Regulation (VAAR) clauses as follows: 62-211-6 Brand Name or Equal; 52.212-1 Instructions to Offerors Commercial Items; 52.212-3 Offeror Representations and Certifications Commercial items; 52.212-4 Contract Terms and Conditions ? Commercial Items; 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Orders Commercial Items with (b)(11), (12), (13), (14), (15), (16), (19)(ii), (21) (22) and (25) checked; and 852.270-4 Commercial Advertising. The provision at 52.212-2, Evaluation of Commercial Items, applies to this acquisition. The evaluation criteria are as follows: (1) technical capability of the item offered to meet the Government requirement and (2) price. The product offered must meet all the salient characteristics specified above to be considered for award. Award will be made to the offeror proposing an acceptable product, price and other factors considered.
