Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation WILL NOT be issued. The solicitation number is RFQ 247-0280-02. This procurement is being conducted under Simplified Acquisition Procedures in accordance with FAR Part 13. The solicitation is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through FAC 2001-07. The VA intends to award a firm-fix priced contract for Register Nurses (RN) for Health Care Resources in accordance with Public Law 104-262 and 38 U.S.C. 8153 to be furnished by the contractor at the Dwight David Eisenhower Army Medical Center (DDEAMC), Fort Gordon, GA for treatment of eligible veterans and other eligible beneficiaries hospitalized in the Cardio Vascular Program for the Augusta VA Medical Center, Augusta, Georgia. PRICE IS AN ALL-INCLUSIVE RATE OF PER DIEM, TRAVEL. CLIN1: Base Period 6/24/02 ? 9/30/02 Description: The contractor shall provide 3 RNs for 19 12-hour tours (approximately 228 hours per week) as described in the statement of work for an estimated 3192 hours. CLIN 2: Option Period 1 10/1/02 ? 3/31/03 Description: The contractor shall provide 3 RNs for 19 12-hour tours (approximately 228 hours per week) as described in the statement of work for an estimated 5928 hours. CLIN 3: Option Period 2 4/1/03 ? 9/30/03 Description: The contractor shall provide 3 RNs for 19 12-hour tours (approximately 228 hours per week) as described in the statement of work for an estimated 5928 hours. STATEMENT OF WORK: Period of performance will be from June 24, 2002 through September 30, 2002 with two (2) additional six-month option periods that may be exercised by the Government. Nurses are required to attend orientation from 8:00 am ? 4:30 pm the first two (2) weeks and then work 12- hour shifts thereafter. REQUIREMENTS FOR: Three (3)-FTE Registered Nurse (RN) ? Critical Care 1. QUALIFICATIONS. RN shall: (a) Hold a diploma, Associates Degree (AD) or Bachelor of Science in Nursing (BSN) degree from an NLN accredited program, college or university and hold a current license to practice as a RN in a state, territory or commonwealth in the United States or District of Columbia. A BSN is preferred. (b) Have been actively practicing as a RN in a critical care setting for the past three years (c) Be capable of providing complex and comprehensive nursing care for adult and pediatric medical and surgical patients. (c) Have and maintain current Basic Cardiac Life Support (BCLS), Advanced Cardiac Life Support (ACLS) certifications, Pediatric Advanced Life support (PALS) certification is preferred. (d) Current CCRN certification or documentation or completion of formal critical care nursing course is preferred. (d) Have documentation of required continuing education in Fire, Safety and Infection Control.(e) Have no health or physical disability restrictions that interfere with the performance of assigned duties. (f) Have documented training and skills verification. (g) Have proof of U.S. citizenship (certified copy of birth certificate or naturalization papers or a valid U.S. Immigration Form 1-151 and Alien Registration Card (?green card?) for each RN. No alien shall be allowed to perform under this contract in violation of the Immigration Laws of the United States. 2. DUTIES AND RESPONSIBILITIES. The RN shall: (a) Provide complex and comprehensive critical care nursing services within the professional scope of practice for a variety of diagnoses for patients of all ages. (b) Conduct initial assessment on patients, develop nursing care plans based on identified patient needs, coordinate with ancillary services to assure provision of care, document all plans, assessments and care provided. Provide patient and family educations throughout the hospitalization. Initiate discharge planning. (c) Administer oxygen, IV fluids, blood transfusion, prescribed medications and titrate medications in accordance with DDEAMC policy. (d) Observe and assess patients? clinical condition, recognize and promptly report significant changes indicative of complications. (e) Initiate resuscitative measures in emergency situations. (f) Operate and monitor specialized equipment including, but not limited to: oxygen, hypothermia units, various types of suction apparatus, infusion equipment, patient controlled analgesia pumps (PAC), epidural pumps, sequential compressions devices, cardiorespiratory monitors, vital sign monitors, invasive hemodynamic catheters and monitors and ventilators. (g) Make nursing care assignments for various skill levels of professional and assistive personnel as assigned. (h) Prepare patients for surgical and diagnostic procedures and assist with procedures as necessary. Provide post- procedure and post-operative recovery care. (i) Ensure that all JCAHO standards for professional nursing care are net during performance of care. (j) Complete appropriate medical record forms in compliance with DDEAMC guidelines and policies on each patient. (k)Verify the content and correctness of all recorded entries in the patient record. All documentation/records/reports must meet or exceed established DDEAMC standards to include, but not limited to: timeliness, legibility, accuracy, content and signature. The outpatient record shall be completed the same the patient is seen. (l)Respond to emergency codes as directed by the Deputy Commander for Nursing or designee and summon appropriate backup from the clinical and hospital staff. (m) Assist in the training and education of nursing students and in the orientation and precepting of assigned personnel. The NAICS Code is 621399 and the size standard is $6.0 Million.
