Description
Quotes are sought for Mental Health Consultation and staff training services at Curlew Civilian Conservation Center (Job Corps), a co-educational vocational training center, in accordance with Job Corps sponsored training. Some training may be required at other locations. Services include furnishing all labor, materials and supplies, including transportation, except as stated in Item #4 below. This is a combined synopsis/solicitation for commercial items prepared in accordance the format in Federal Acquisition Regulations (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-07. This acquisition is a small business set aside. NAICS code is 624190 with small business size being $6.0 million annual ave. gross income. The average Curlew Job Corps student population is around 200. Student ages range from 16 through early 25?s. The Center is located in north of Spokane, Washington, via state highway 395 through Colville and Kettle Falls, to its junction with highway 20; then, via highway 20 (Sherman Pass) to Republic; then north on highway 21 to Curlew, WA (just a few miles from the US/Canada border), then travel 10 miles Northwest of Curlew on County Road 501. Quotes should be sent delivered in writing directly to Jerry Case, Purchasing Agent, 509-779-7646. The following are the requirements and estimated hours for each ordering year (July 1st through June 30th for years 2003, 2004, 2005, and 2006): Item 1 - MENTAL HEALTH COUNSELING AND CASE EVALUATION - 200 hours: A. Individual counseling to problematic students, documentation, and recommendations to enter staff in working with students, crisis intervention, emergency psychiatric consultation, and referral. B. Mental health evaluation for individuals to determine treatment requirements, diagnostic information, and to provide Counselor/Center feedback as to individual suitability for the Job Corps program. C. Consult with staff TEAP (Trainee Employee Assistance Program) Specialist regarding student substance abuse evaluations and issues. Item 2 - REVIEW OF STAFF REFERRALS - 150 hours: Review referrals for counseling from other sections on center with counseling staff to determine action plans, staffing, and appropriate handling of student referral. Item 3 - STAFF TRAINING - 85 hours: Includes training of Job Corps staff to enhance ability to perform effective instruction to the students. Item 4 - ATTEND JOB CORPS SPONSORED TRAINING - 15 hours: Necessary training that the Job Corps deems needed by the contractor to better serve the student population and understand Program directions. This item includes reimbursement of per diem and mileage. The Contractor shall furnish all equipment needed for performance of the work of this contract. Most services are to be performed on site in Government provided offices. All services must meet all applicable state laws and municipal standards. The contractor shall be capable of performing to the current industry methods and standards. Services shall be available at least 10 hours per week for approximately 45 weeks. The actual days and times of service, will be negotiated upon commencement of services. Resulting award will be an indefinite delivery, indefinite quantity contract for four years. Contract guarantee is 450 hours with an estimated 4-year total of 1800 hours. Provision at FAR 52.212-1, Instructions to Offerors Commercial, applies to this acquisition along with the following addenda of submittal requirements: Offerors are to provide a full resume of qualifications and experience including references (names telephone numbers, and type of work performed), and hourly rates for each item listed above. Offerors should submit a quote in their own forms based on the information provided herein. Offerors should also provide a completed copy of the provision at FAR 52.212-3, offeror Representation and Certifications Commercial Items. Award will be based on quotes received considering price (hourly rates) and capability. Capability includes factors such as qualifications, experience in performing similar work, past performance, dependability and cooperation. Price and capability will be considered together, with equal weight, to determine the best value to the Government for award.
