- Per FAR 5.207, the following synopsis information is provided: 17. Description: (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Request for Quotation, F32604-02-T0033. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-07. (iv) Notice: This solicitation is 100% set-aside for small business concerns. The North American Industry Classification is 333921. The size standard is 500 employees (v) Items to be procured: ITEM 0001: Elevator Maintenance - contractor will provide all parts, labor, tools, materials, and transportation necessary to accomplish elevator maintenance at Minot AFB ND in accordance with the Statement of Work provided from 1 Oct 02 through 30 Sep 03. All necessary utilities will be provided by the government. (vi) Additional description of line item 0001 can be found in attached Statement of Work (vii) Required delivery shall be 1 Oct 02 through 30 Sep 03. Place of delivery shall be 5 CES, 320 Peacekeeper Place, Minot AFB ND 58705. FOB: Destination. (viii) Solicitation provision and clauses applicable to this action are attached. Refer to the Quote Sheet/Reps & Certs/Provisions & Clauses attachment. (ix) Evaluation will be based on price and price related factors. (x) Include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items (Alt I and III), with quote. An electronic version is available for download along with a quote sheet at http://www.eps.gov (xi) The Defense Priorities and Allocations System (DPAS) rating is: unrated (xii) Mail or fax complete quote package to 5 CONS/LGCA-2, Attn Jeff Strange, 211 Missile Avenue, Minot AFB ND 58705-5027.
