Description

The 30th Space Wing, Vandenberg AFB CA, is soliciting offerors for Dental Hygienist for the 30TH Medical Group on Vandenberg AFB, CA. This is a combine synopsis/solicitation for a commercial item prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotes are being requested and a written solicitation will not be issued. The solicitation number is F04684-02-T-0066 and will be issued as a request for quotation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition circular 20001-07. This acquisition is a 100 % set-aside for small business participation only. The SIC Code is 8049 and size standard is 5 million. There will be one contract line item, one job, for this acquisition. The contractor is to provide dental hygienist duties in accordance attach Statement of Work. ALL FAR CLAUSE ARE LOCATED AT: http://farsite.hill.mil/farsite. The Provisions at FAR 52.212-1 "Instructions to Offerors-Commercial,"with the following addenda applies to this acquisition: Offerors shall submit past performance data consisting of three recent and relevant contracts with their offer. The provisions at FAR 52.212-2 "Evaluation-Commercial Items" applies to this acquisition. Insert in Para (a), "The following factors will be used to evaluate offers, (1) price and (2) past performance. Offerors must submit with their quotes a completed copy of the provisions at 52.212-3 "Offeror Representations Certifications-Commercial Items" The clause at FAR 52.212.4 Contract Terms and Conditions Commercial Items, applies to this acquisition along with the following addenda: FAR 52.219-8 "Notice to Total Small Business set-aside.
