Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; PROPOSALS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-07. This procurement is UNRESTRICTED. The North American Industry Classification System Code is 562211, Hazardous Waste Collection and Disposal Services. CONTRACT LINE ITEMS:CLIN 0001, Base Year: Chemotherapy waste collection and disposal. estimated 20 each 50 lb. containers per month. Rate per container:________. CLIN 0002, Option Year 1: Chemotherapy waste collection and disposal, estimated 20 each 50 lb. containers per month. Rate per container: ________. CLIN 0003, Option Year 2: Chemotherapy waste collection and disposal, estimated 20 each 50 lb. containers per month. Rate per container: ________ CLIN 0004, Option Year 3: Chemotherapy waste collection and disposal, estimated 20 each 50 lb. containers per month. Rate per container: ________. CLIN 0005, Option Year 4: Chemotherapy waste collection and disposal, estimated 20 each 50 lb. containers per month. Rate per container: ________. STATEMENT OF WORK. GENERAL: The Contractor shall provide labor, transportation, collection, removal, and destruction for the disposal of Chemotherapy Waste (which is a Regulated Medical Waste) generated by the VA Salt Lake City Health Care System (VASLCHCS) as specified in the Schedule above and in accordance with the terms and conditions herein. DEFINITIONS: Regulated Medical Waste: A waste or other reusable material that contains an infectious substance and is generated in the diagnosis, treatment, or immunization of human beings or animals; research pertaining to the diagnosis or treatment of humans or animals, and the production or testing of biological products. The term Regulated Medical Waste, as used herein, shall include only those wastes defined by the United States Environmental Protection Agency (USEPA) as infectious wastes in its Guide for Infectious Waste Management (#EPA/530-SW-86-014, May 1986) and any other wastes identified as infectious or similar wastes in any other applicable federal, state, country or municipal laws, regulations, guidelines and Chemotherapy Waste. Regulated Medical Waste also refers to the following categories of waste which require special handling: (1.) Liquid or semi-liquid blood or Other Potential Infectious Material (OPIM); (2.) Items contaminated with blood or OPIM, which would release these substances in a liquid or semi-liquid state if compressed; (3.) Items that are caked with dried blood or OPIM and are capable of releasing these materials during handling; (4.) Contaminated sharps; (5.) Pathological and microbiological wastes containing blood or OPIM; (6.) Items 1-5 contaminated with chemotherapy waste. Chemotherapy Waste: Chemotherapy Waste as used herein, means discarded items which have been contaminated by chemotherapy drugs or antineoplastic agents, provided that such items (including vials, syringes, IVs and related tubing, and sharps) shall be empty or contain only trace amounts of chemotherapy drugs as are allowable under applicable federal, state, county or municipal laws, regulations and guidelines so as to be considered waste. The term Regulated Medical Waste specifically excludes corrosive, reactive, radioactive, toxic (except toxic waste listed as an antineoplastic agent in Section 10.00 of the American Hospital Formulary Service published by the American Society of Hospital Pharmacists) and other Regulated Medical Wastes and substance as defined in any applicable federal, state, country or municipal laws, regulations, and guidelines. PICKUP LOCATIONS: The pickup locations for the Chemotherapy Waste shall be two locations at the VASHLC. These locations are: (1.) Bldg. 1, Room 1C27C, and (2.) Bldg. 14, Room BC05. PICKUP PROCEDURE: Pickup will be required on an ?on call basis? at the VASLCHCS. The VASLCHCS Safety Officer will notify the contractor when a pickup is required and arrangements will be made to have the Safety Officer and EMS personnel available during pickup of the Chemotherapy Waste. Pickups will not be performed without the Safety Officer present and EMS personnel available. REGULATIONS: All work shall be accomplished in accordance with Federal, State, and local regulations. The applicable federal regulations are: (1.) Bloodborne Pathogen Standard (29 CFR 1910.1930) and (2.) Hazard Communication Standard ( 29 CFR 1910.1200). STORAGE: The VASLCHCS will store the Chemotherapy Waste in containers supplied and approved by the Contractor. The storage of Chemotherapy Waste shall be confined to areas to which only authorized personnel have access. The VASLCHCS will be responsible for properly and lawfully packaging and storing the Chemotherapy Waste. TRAINING: The Contractor shall provide training to the employees at the VASLCHCS on the proper packaging of chemotherapy waste prior to the first pickup under the purview of this contract. CONDITIONS OF COLLECTION/TRANSPORT/TREATMENT/DISPOSAL: Contractor reserves the right to decline to accept for collection, transportation, treatment and disposal any Chemotherapy Waste which cannot be transported, treated, or disposed of in a lawful manner or without risk to human health or the environment. Improperly packaged, leaking, overweight, or damaged containers will not be accepted for pickup by the contractor. If the container must be repackaged by the Contractor, it will only be done under the supervision of the VASLCHCS Safety Officer and will be reimbursable. TITLE TO CHEMOTHERAPY WASTE: Title to the Chemotherapy Waste collected from the VASLCHCS shall be transferred to and vested to the Contractor at the same time that the Chemotherapy Waste is fully loaded into the Contractor?s truck. Prior thereto, all title to the Chemotherapy Waste shall be the responsibility of the VASLCHCS. Title to any waste other than Chemotherapy Waste shall remain with the VASLCHCS. CONTRACTOR REQUIREMENTS: The Contractor shall provide the services enumerated above at the rates set forth which include all applicable taxes, assessments, and related charges. The Contractor will provide all of the necessary equipment to perform such services and all such equipment shall comply with all applicable federal, state, county and municipal laws, regulations, guidelines, and permits. Any changes to type, size, amount of equipment required to perform services, and the frequency of service may be mutually agreed to in writing by both parties without affecting the validity of the contract. EVALUATION FACTORS FOR AWARD: FAR 52.212-2 Evaluation ? Commercial Items (Jan 1999) (a) The government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered: FACTOR 1: Demonstrated technical capability. FACTOR 2: Past Performance. FACTOR 3: Cost. Technical and past performance, when combined, are given more weight than cost. Documentation of technical capability and past performance is required. INVOICING: Instructions for invoicing will be provided in the Notice of Award letter to the successful offeror. CONTRACTING OFFICER?S TECHNICAL REPRESENTATIVE (COTR): The identity and contact information for the COTR will be provided in the Notice of Award letter to the successful offeror. CONTRACT PROVISIONS. Offerors are required to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items (MAY 2002), with its offer.
