- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number RFQ 600-054-02 is being issued as a request for quotation (RFQ) which incorporates provision and clauses through Federal Acquisition Circular 2001-06. This solicitation is a total set-aside for small business concerns and only quotations from small business concerns will be considered. The NAICS code is 621399 and the small business size standard is 7.5Mil. The anticipated base year performance from 7-3-02 to 7-2-03 with renewal options for up to four (4) additional years. Contractor shall quote pricing on an hourly rate basis. Line item #1 Provide on site ARRT licensed Diagnostic and Diagnostic CT Technologists service for an estimated 3000 hours. Line item #2 Provide on-site ARDMS licensed Ultrasound Technologists service for an estimated 600 hours. Contract ARRT and ARDMS Technologists to augment in house staffing shortages during the normal hours Monday through Friday 8:00 a.m. to 4:30 p.m. or 10:00 a.m. to 6:30 p.m., excluding legal holidays at the VA Long Beach Healthcare System, 5901 East Seventh St., Long Beach, CA 90822. The anticipated requirements for the contract period will vary depending on staffing and patient level, however, the VA will provide the contractor at least one week advance notice whenever possible. Emergency coverage by the CT Technologist and Ultrasound Technologist is to be provided on an as needed basis during after hours, weekends and holidays. Emergencies may require short notice of (24) hours. It should be noted that overtime in excess of 8 hours per day is not expected, however, should a patient procedure continue past the end of shift, the technologist will be requested to complete the procedure. Overtime rate of pay in excess of the shift will be one and one/half the basic rate. Although the initial list of participants includes only the VA Long Beach Healthcare System, the Government reserves the right to add additional VA facilities. CLINICAL DUTIES AND RESPONSIBILITIES: (a) ARRT Diagnostic/CT Technologists perform routine procedures of the bones, joints, spines and skull. Must be capable of performing all diagnostic studies. In addition, must be able to perform UGI’s Bes, and IVP’s in accordance with the facility protocol. For CT experience, must possess documented experience with operating and use of Computed Radiolgraphy (CR). If not, vendor must be willing to train technologists at no expense to the VA. Diagnostic Technologists experience on Digital fluoro and Direct Radiography (DR) is preferred. (b) ARDMS Ultrasound Technologist duties include general sonography procedures of Abdomen, Liver, Gall Bladder and DVT’s. Perform vascular duplex color doppler evaluations of carotid arteries and deep venous thrombosis. Must be able to operate Acuson, Hitachi and ATL equipment. (c) ARRT Technologist and ARDMS Ultrasound Technologist (Registry Technologists) will be under the direct supervision of the Supervisor Radiology Service. (d) The contracted Registry shall screen all technologists before making referrals in order to determine the qualifications and competence of said personnel. Competency requirements will be defined by VA Long Beach Healthcare System. (e) The VA reserves the right to examine, approve or disapprove individuals based upon information provided in credentials and/or resumes. (f) American Registry of Radiologic Technologists and American Registry of Diagnostic Medical Sonographers shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia and required to show proof of licensure and carry that license during the assignment to the VA. (g) The operation of services contracted for at the VA will be provided by the VA at mutually agreed upon levels which are compatible with the safety of the patient and personnel and quality medical care. (h) Contract personnel shall be responsible for compliance with all procedures in accordance with applicable VA Healthcare System written policies, procedures and protocols. (i) The successful contractor shall commit to providing a backup locum ARRT and ARDMS technologists who possess the same qualifications as those required for the primary technologists. (j) Person provided by the Contractor shall be technically proficient in the skills necessary to fulfill the Government’s requirements, to include the ability to speak, understand, read and write English fluently. (k) Contractor shall gather information on patients screened and report findings. (1) Personnel Policy: The contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out these responsibilities the contractor shall provide the following: (1) Medical Liability Insurance. (2) Worker’s Compensation, (3) General Liability Insurance. (4) Income Tax Withholding (5) Social Security Payments. The parties agree that such personnel shall not be considered VA employees for any purpose and shall be considered employees of the contractor. QUALIFICATION: Only qualified contractors will be considered and evidence of qualifications and experience must be submitted. 1) Technical Ability: (a) A detailed plan shall be submitted describing the firm's ability to support the Technologists. The narrative shall show evidence of reliability, financial responsibility, and credibility. This plan shall include the locations of offices, phone numbers, number of staffing, number of years in business and managerial plan. (b) Agency enforcement of standards of review and evaluation to ensure ongoing quality service from health care professionals. (c) Provide contingency plans to be used in the event of unexpected absences or required replacement due to rejection of contract personnel. Contractor must maintain a minimum of three trained and acceptable ARRT and ARDMS technologists for each category. (d) Provide an employee's resumes including experience, expertise, education, background and any unique skills for at least three qualified technologists available from your registry pool. (e) Contractor must provide documentation showing recent (annual) TB tests and BLS certificates. (f) Diagnostic/CT Technologist must be ARRT certified, have a minimum of 3 years experience working in a hospital setting as a Diagnostic Radiology Technologist. Experience in operation of Computed Radiograph (CR) equipment expected (g) ARDMS certified Ultrasound Technologist must be specialized in abdominal organs and have a minimum of 2 years experience working in a hospital setting as a Ultrasound Technologists. (h) Provide documentation showing evidence of complete background search that technologists are bonded and insured by the agency. 2) Past Performance: Contractor shall submit a list of (3) current or recently completed contracts performed within the past year. As a minimum, contracts listed may include those entered into with the Federal Government, State or local agencies or commercial customers. For each contract listed, provide the name of customer organization; contract number; contract value; and name, address, and phone and fax numbers of the customer. (i ) All documentation must be submitted to the Radiology Service prior to any technologist being assigned to this facility. All required orientation and/or training must be completed to the VA’s satisfaction prior to the technologist being assigned to the site. CONTRACTOR PERSONNEL SECURITY REQUIREMENTS: The Office of Security and Law Enforcement provides Department-wide policy on the assignment of appropriate position sensitivity designations associated with Department of Veterans Affairs (VA) positions involving national security and public trust responsibilities, and on the level of background investigations required for applicants for, and incumbents of, those positions. In addition to VA employees, the policy and investigative requirements are applicable to contractor personnel who require access to VA computer systems designated as sensitive. Contractor and subcontractor personnel who require access to VA computer systems shall be subject to all necessary background investigations and receive a favorable adjudication from the VA Office of Security and Law Enforcement to ensure compliance with such policy. If such investigation has not been completed prior to contract commencement, the Contractor shall be responsible for the actions of those individuals performing under the contract. Should the contract require Contractor personnel to maintain U.S. citizenship, the Contractor shall be responsible for compliance. Regardless of U.S. citizenship requirements, Contractor personnel are required to read, write, speak, and understand the English language, unless otherwise specified in this contract or agreed to by the Government. The cost for such investigations shall be borne by the Contractor, either in advance or as reimbursement to the Government. The level of sensitivity shall be determined by the Government on the basis of the type of access required. The level of sensitivity will determine the depth of the investigation and the cost thereof. At this time, the current estimated costs for such investigations is as follows: LEVEL OF SENSITIVITY BACKGROUND INVESTIGATION LEVEL APPROXIMATE COST Low Risk National Agency Check with Written Inquiries $ 200.00 Moderate Risk Minimum Background Investigation $ 795.00 High Risk Background Investigation $ 2,765.00 The Contractor shall be required to furnish all applicable employee information required to conduct the investigation, such as, but not limited to, the name, address, and social security number of Contractor personnel. The VA will provide all the necessary instructions and guidance for submission of the documents required to conduct the background investigation. Background investigations shall not be required for Contractor personnel who will not be required to access VA computer systems nor gain access to sensitive materials. EVALUATION FACTORS: 52-212-2 Evaluation-Commercial Items (JAN 1999): Offeror’s experience will be evaluated as it relates to the services requested. The offeror whose quotation represents the best value to the Government will be awarded. Quotations will be evaluated on the following criteria, ranked in order of importance. Factor I: Technical Ability of firms and experience of the proposed locum technologists, Factor II: Past performance, Factor III: Price. All evaluation factors when combined are slightly more important than price. This procurement is subject to availability of funds. All offers shall be clearly marked with the offerors name and RFQ number. Only offers completely prepared in accordance with this solicitation will be considered.
