Description
This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This procurement is being conducted under the Simplified Acquisition Procedures (SAP). This procurement is a total small business set-aside. The NAICS Code and the small business size standard for this procurement are 812331 and $12M, respectively. The quoter shall state in their quotation their size status for this procurement. NASA Johnson Space Center in Houston, Texas, is requesting quotes for linen rental and laundering services for JSC civil service lab personnel. Linens to be rented and cleaned include: coveralls, lab coats, aprons, smocks, shirts, and towels as specified in the attached model contract and statement of work. Estimated quantities for the above items can be found in the attached model contract, Part 11: Exhibit A (Contract Pricing), which is the Government's best estimate of services required. The contractor will be given formal notification during the period of performance of any additions or deletions to the quantities listed in Exhibit A. Offerors are requested to provide prices for any ancillary items they feel are required to perform this effort which are not included in Exhibit A. The Government will make the final determination as to what items are in its best interest to procure in performance of this contract. Linens are to be picked-up/delivered to labs located in approximately seven buildings on the JSC site (including the Neutral Buoyancy Lab) as listed in the attached model contract, Part 12: Exhibit B (Pickup and Delivery Points). If a site visit is desired, please contact the procurement points of contact referenced herein This contract will be awarded for a one (1)-year base period with two (2)-year option periods, for a total of five (5) years. Offerors are required to submit quotes for the base period and options years in accordance with Exhibit A. It is anticipated that the contract will begin on or before August 1, 2002. The provisions and clauses in the RFQ are those in effect through FAC 01-7. All qualified responsible business sources may submit a quotation which shall be considered by the agency. Delivery shall be FOB Destination. The DPAS rating for this procurement is DO-C9. Quotes and associated documentation as described in this solicitation and its attachments are due by 4:00 p.m. (CST) on Monday, June 26,2002, and may be mailed or faxed to Michelle Diefenderfer, Mail Code BJ2, 2101 NASA Rd. 1, Houston, TX 77058; Fax 281-483-4173 and include: solicitation number, FOB destination to this Center, proposed delivery schedule, discount/payment terms, warranty duration (if applicable), taxpayer identification number (TIN), identification of any special commercial terms, and be signed by an authorized company representative. Quoters are encouraged to use the Standard Form 1449, Solicitation/Contract/Order for Commercial Items form found at URL: <u> http://procure.arc.nasa.gov/Acq/Forms/Index.html </u> to submit a quotation. In addition to the items described herein, refer to the attached "Instructions for Offerors" for a list of required documents that must be furnished to be considered responsive to this solicitation. Quoters must provide copies of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items with their quote. The Representations and Certifications may be obtained via the internet at URL: <u> http://ec.msfc.nasa.gov/msfc/pub/reps_certs/sats/ </u> If the end product(s) quoted is other than domestic end product(s) as defined in the clause entitled "Buy American Act -- Supplies," the quoter shall so state and shall list the country of origin. FAR 52.212-4 is applicable. FAR 52.212-5 is applicable as outlined in the attached model contract. Evaluation of proposals will be conducted based on price, technical acceptability, past performance, and estimated transition period required. Price determination will include evaluation of the base period pricing as well as options I and II pricing. Technical acceptability will be determined using information submitted by the quoter providing a description in sufficient detail to show that the product quoted meets the Government's requirement. Vendor must furnish a minimum of three (3) references that include points of contact for past performance evaluation. An ombudsman has been appointed.
