Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTATIONS ARE BEING REQUESTED AND A WRITTEN SOLICITATI0N WILL NOT BE ISSUED. This solicitation number RFQ600-054-02 is being issued as request for quotation (RFQ) which incorporates provisions and clauses through Federal Acquisition Circular 2001-07. The Network Business Center (NBC/SP) at the VA Long Beach Healthcare System, 5901 East Seventh St., Long Beach, CA 90822 is soliciting for a requirement at the VA Long Beach Healthcare System to provide on-site ARRT licensed Diagnostic and Diagnostic CT Technologists service for an estimated 3000 hours annually AND on-site ARDMS licensed Ultrasound Technologists service for an estimated 600 hours annually. Contractor shall quote pricing on an hourly rate basis. Contract ARRT and ARDMS technologists will augment in house staffing shortages during the normal hours Monday through Friday 8:00 a.m. to 4:30 p.m. or 10:00 a.m. to 6:30 p.m., excluding National holidays. This solicitation is a 100% Total Small Business Set Aside and only quotations from small business concerns will be considered. The NAICS code is 621399 and the small business size standard is $7.5Mil. The anticipated base year contract period is for one year from date of award with up to four (4) additional one-year options. The anticipated requirements for the contract period will vary depending on staffing and patient level, however, the VA will provide the contractor at least one week advance notice whenever possible. Emergency coverage by the CT Technologist and Ultrasound Technologist are to be provided on an as needed basis during after hours, weekends and holidays. Emergencies may require short notice of (24) hours. Should a patient procedure continue past the end of shift, the technologist will be required to complete the procedure. Overtime pay in excess of the shift will be one and one/half the basic rate. Although the initial list of participants includes only the VA Long Beach Healthcare System, the Government reserves the right to add additional VA facilities. EVALUATION FACTORS FOR AWARD: FAR 52.212-2 Evaluation-Commercial Items (JAN 1999). The government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. Quotations will be evaluated on the following criteria, ranked in order of importance: Factor I: Technical Ability of firm and experience of the proposed locum technologists. Factor II: Past performance. Factor III: Price. Technical and Past Performance, when combined, are given more weight than price. The following incorporated clauses and provisions are applicable to this solicitation: Offerors are required to submit a completed copy of the provision at FAR Clause 52.212-3 Offerors Representation and Certifications- Commercial Items (MAY 2002) and a DUNS number with their quote. The Government reserves the right to award without discussions. Therefore, each initial quotation should contain the offeror?s best terms and conditions from a price and technical standpoint. This procurement is subject to availability of funds. All offers shall be clearly marked and include the RFQ number, requesters name, address, telephone and fax numbers. Only offers completely prepared in accordance with this solicitation will be considered.
