- Description
- Previously synopsized 18 September 2001. Provide multidiscipline healthcare providers at Coast Guard clinics located in WA, OR, CA, HI, and AK. This will be a personal and non-personal services contract. Non-personal services will require medical malpractice insurance in the amount of $1 mil. Services to be provided are as follows: Registered Nurses, Dental Assistants, Family Nurse Practitioners, Physician?s Assistants, Registered Dental Hygienists, Registered Physical Therapists, Optometrists, Flight Surgeons, Physicians, Dentists and Medical Record Technicians. This requirement will be accomplished utilizing Commercial Item Best Value procedures. Written proposals and oral/visual presentations will be required. Contract providers will work weekdays during hours established by the Clinic Supervisors, part time and full time. Period of performance will start 01 October 2002, or date of award through 30 September 2003. Four additional annual option years are available and four possible additional annual award terms. A pre-bid conference will be held approximately 15 days after issuance of the solicitation to answer questions. The Request for Proposals will be issued on or about 15 March 2002 and close on or about 30 April 2002. This procurement is a Small Business Set-Aside. The NAICS code for this acquisition is 621399, the SIC code is 8049 and the small business size standard is $6.0 million average annual receipts for the proceeding three fiscal years. All responsible sources may submit a bid, which shall be considered by the agency. (See numbered note #1.) The solicitation will be available through FEDBIZOPPS website at www.eps.gov. Solicitation hard copies will not be mailed. ATTENTION: Minority, Women-Owned and Disadvantaged Business Enterprises (DBEs) The Department of Transportation (DOT), Short Term Lending Program (STLP) offers working capital financing in the form of lines of credit to finance accounts receivable for transportation related contracts.
