Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is RFQ-32-08-02-ORB-2321. It is anticipated the requirement will not exceed the Simplified Acquisition Threshold. The requirement is for a fixed priced contract. The solicitation is issued as a Request For Quote (RFQ) for professional mental health services at the Flatwoods Civilian Conservation Center in Coeburn, Virginia. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-07. The associated NAICS code is 621112. The small business size standard is $8.5 million. The solicitation is for one base period and two, one-year options. The Government will provide (1) sufficient space for student consulting, staff consulting and training; (2) access to information necessary to provide services, such as records, directives, reports, Job Corps Regulations, or other relevant information to include the availability of personnel and specialists for technical consultation as needed for the orderly and efficient development of services. DESCRIPTION OF REQUIREMENT: Please state your price per hour for base year beginning July 15, 2002 and ending June 30, 2003 with an estimated 416 hours, Option Year 1 for period beginning July 1, 2003 and ending June 30, 2004 with an estimated 416 hours and Option Year 2 for period beginning July 1, 2004 and ending June 30, 2005 with an estimated 416 hours. Contractor shall furnish professional mental health services to students at Flatwoods Civilian Conservation Center, Coeburn, Virginia. Services will be provided weekly on alternating Tuesdays (11:00 AM to 5:30 PM) and Thursdays (12:00 Noon to 6:30 PM). Services shall include but will not be limited to those mental health services specified in the Job Corps Federal Regulations (Title 20, Code of Federal Regulations 638.510, the Job Corps Policy and Requirements Handbook, Chapter 6, and Technical Assistance Guides 6-D, 6-L 6N and 6-Q. Services to be provided include: (1) Staff Development (minimum 1 hour per 100 students per week): (a) Advice to the Center Director and Health Services Staff on all mental health matters and overall center program and organizational consultation to the Center Director and Administrative Staff on a regular basis; this will include staffing requirements. (b) Consultation on the development of a system to promote the coordination and integration of the mental health program with other center programs and activities such as counseling, residential life, and recreation. This will be done in cooperation with the health services manager, center physician, center dentist, other health professional, as well as any staff member involved with students. (c) Advice and assistance to health education instructors on the implementation of the health education program. (d) Consultation with center health staff, counselors, residential advisors, instructors, and other appropriate staff concerning the on going mental health problems of students. (e) Assistance with program development for the purpose of meeting the life crises of students in constructive way. For example, involvement with the center disciplinary program, the maximum benefits program, the intergroup relations program, and the AODA program. (f) Staff development on using mental health principles and techniques. This will include coping with the problems of alcohol and drug use, sexuality, aggressive behavior and poor communications. (2) Clinical Services (minimum 1 hour per 100 students per week): (a) Advice to the Center Director on the advisability of students terminations for mental health reasons, and the cost of mental health services, include inpatient facility use. (b) Ensurance of the maintenance, confidentiality, and safeguard of all necessary mental health records. (c) Issuance and maintenance of signed and dated standing orders regarding mental health matters in accordance with the Technical Assistance Guide 6-Q, (d) Diagnostic and evaluative services to students who are inpatients or outpatients. (e) Limited forms of psychiatric treatment including crisis intervention brief psychotherapy, intermittent supportive psychotherapy, psychotropic drug management and group psychotherapy, as permitted by licensure or accreditation. (f) Assisting with appropriate referral for students who must be terminated for mental health reasons. (g) Providing clinical assistance to the AODA specialist. The professional and the center agree that the professional will have primary responsibility under the Center Director's guidance to implement the program outlined above. The professional with the concurrence of the Center Director, may delegate by written and dated standing orders any of his/her responsibilities set forth in the subcontract to appropriate qualified center staff. The professional will be assisted by the Job Corps National Health Staff (NHS) and principal and regional health consultants. The latter will act under guidance and direction from the NHS. Under this agreement, the professional is an independent contractor and not an employee or agent of the center contractor or the Department of Labor, Job Corps. The professional agrees to indemnify and save the center operator harmless from any expenses including attorney's fees and also claims on account of damage to property or bodily injury (including death) which may be sustained by himself in connection with work performance. The professional agrees to make no public statements concerning Job Corps students or Job Corps activities without prior written approval of the Center Director or his designated representative. The Contractor shall provide proof of malpractice insurance for any persons performing services under the terms of this contract. Contractor shall provide a copy of current certification, licensing or accreditation in the State of Virginia. PROVISIONS AND CLAUSES: The following provisions and clauses apply: 52.212-1, Instructions to Offerors-Commercial Items, 52.212-2, Evaluation-Commercial Items, applies with paragraph (a) completed as follows: Award will be made to the offeror that meets the solicitation?s criteria for price, experience and past performance. Offeror will provide documentation of qualifications and experience with solicitation package, 52.212-3, Offeror Representations and Certifications-Commercial Items (MUST BE RETURNED WITH SOLICITATION PACKAGE, 52.212-4 and Contract Terms and Conditions-Commercial Items. FAR 52.212-5, Contract Terms and Conditions Required To Implement Statues or Executive Orders-Commercial Items, applies with the following applicable clauses for paragraph (b): 52.222-21, Prohibition of Segregated Facilities, 52.222-26, Equal Opportunity, 52.222-35, Equal Opportunity for Special Disabled Veterans, Veterans of the Vietnam Era, and Other Eligible Veterans, 52.222-36, Affirmative Action for Workers with Disabilities, 52.222-37, Employment Reports on Special Disabled Veterans, Veterans of the Vietnam Era, and Other Eligible Veterans, and 52.232-34, Payment by Electronic Funds Transfer-Other than Central Contractor Registration. The below additional contract requirement(s) or terms and conditions apply: 52.217-8, Option to Extend Services, fill in information, (within 60 days of the end of the performance period), 52.217-9, Option to Extend the Term of the Contract, fill in information (30 days prior to expiration of the contract), (60 days before the contract expires), and (exercise of any options under this clause, shall not exceed three years) and 52.228-5, Insurance Work on a Government Installation.
