- (i) This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Request For Quotation, F5HOSP21140001. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-07. (iv) Notice: This solicitation is a 100% set-aside for small business concerns. The North American Industry Classification is 621999? All Other Miscellaneous Ambulatory Health Care Services. The size standard is $8.5 M. (v) Items to be Procured: Require services, non-personal, period of performance from date of award to one year period for contract auditor/coder. Attached statement of work lists qualifications needed for position. All information for the subject solicitation is available at www.eps.gov, no paper solicitations or specifications will be made available. (vi) There is no additional description for the preceding line item. (vii) Required start date shall be as quickly as possible. Place of employment shall be Medical Treatment Facility, 10 Missile Ave, Minot AFB, ND 58705; FOB Destination. (viii) Solicitation provision at FAR 52.212-1, Instructions to Offerors?Commercial Items (OCT 2000) is hereby incorporated by reference with no addenda to the provision. (ix) Evaluation will be based on price and price related factors.
