Description
The Department of Veterans Affairs, VISN 9, Mid South Veterans Healthcare Network (VA) has an ongoing program to furnish artificial limbs, orthotic appliances and related services to those veterans that require them and are eligible for assistance. It is VA?s intention to solicit proposals and award multiple contracts on a competitive basis to preferred providers within the following VA Medical Center?s (VAMC) jurisdiction under the authority of Title 38 U.S.C. Section 8123. Orthotic appliances include custom made/custom fit braces, corset/belts, elastic hose, arch support, shoe inlays, molded shoes, and orthotic shoes. Initially this acquisition shall be for the Memphis VA Medical Center, however, the other 6 facilities in VISN 9 can be added without amendment at any time. The locations of the facilities are: Huntington VA Medical Center, Huntington, WV, Lexington VA Medical Center, Lexington, KY, Louisville VA Medical Center, Louisville, KY, Memphis VA Medical Center, Memphis, TN, James H. Quillen VA Medical Center, Johnson City, TN, Nashville VA Medical Center, Nashville, TN, Alvin C. York VA Medical Center, Murfreesboro, TN. The Contractor shall fabricate all artificial limbs in strict conformance to the prosthetic prescription that has been provided by the VA Amputee Clinic Team. The Contractor shall fabricate all orthotic appliances in strict conformance to the prescription that has been provided by the VA Physician or authorized fee base physician. Contractors shall not add or substitute components or alter the beneficiaries? limb prescription in any way without prior approval and written authority from the VA Amputee Clinic Team Prosthetic Representative or designee. Contractor shall not add or substitute components or alter the beneficiary?s prosthetics prescription in any way without prior approval and written authority from the VA prescribing physician, prosthetic representative, or designee. VA will award no more than 10 (ten) firm fixed price indefinite quantity contracts pursuant to this solicitation for commercial items per the geographical area of each facility. It is expected that each contract will contain one (1) base year and four (4) renewable option years. VA shall determine (based on an integrated assessment of each Offeror?s technical and price proposals) the individual proposals judged to be most advantageous to the Government. The minimum total dollar amount for supplies and services is estimated to be $4,000. The maximum total dollar amount for the supplies and services specified in this solicitation is estimated to be $3,000,000.00. These minimum and maximum are the estimated amounts of supplies/services the Government intents to acquire under this indefinite-quantity contract.
