Description
The Centers for Disease Control and Prevention (CDC) is seeking proposals to assist in evaluation activities related to the CVH State Program. Specifically, the contract will involve evaluation assistance in the following areas: (1) providing technical assistance to CVH Program staff and CVH States in the area of program evaluation, (2) providing evaluation training to CVH States in a manner that is consistent with the CDC Evaluation Framework and the CVH Evaluation manual, and (3) providing technical assistance to the evaluation lead and CDC CVH Program staff in developing the evaluation component of the CVH Program which includes assisting in revisions of the program logic model, the Evaluation Framework, the Evaluation manual, evaluation of CVH interventions and conceptualizing the overall evaluation strategy for the CVH Program. The major goals of the CVH State Program Evaluation are to: (1) Document changes in State capacity to address CVD and CVH; (2) Synthesize surveillance data on CVD; (3) Document changes in heart-healthy policies and environmental supports; (4) Document the process and impact of State and program interventions; and (5) Document changes in disparity between priority populations and the general population. The period of performance is 12 months, with four 12-month option periods, for a total anticipate period of performance of 60 months. A cost-plus-fixed-fee type contract is contemplated. The NAICS code for this requirement is 541690, and the small business size standard is $6M. This requirement will be done under full and open competition. The estimated release date of the RFP on the http://www.fedbizopps.gov website is on or about July 1, 2002. Hard copies of the RFP will not be mailed.
